Non-Pharmacologic Management of Epilepsy

Abstract: Non pharmacological treatment, in addition to pharmacological treatment is indicated in patients with refractory/pharmacoresistant epilepsy and includes ketogenic diet, deep brain stimulator, vagal nerve stimulator, transcranial magnetic stimulation and epilepsy surgery. Ketogenic diet has been recommended since 1921 and has been proved to be a safe and effective treatment for intractable epilepsy. Deep brain stimulator, has been used in the treatment of movement disorders for many years and recently been trie… Show more

“…In either case, when RTFM is performed, the ECoG signals recorded from the subdural grids (implanted as a part of standard presurgical evaluation for epilepsy surgery) (►Fig. 3) are separated into two streams: (1) video-ECoG monitoring for seizure localization and (2) for RTFM mapping. This is done to ensure that the recording of the ECoG signal for seizure monitoring remains uninterrupted.…”

“…1 Epilepsy surgery can provide with a successful treatment option for this patient population. 2,3 Successful seizure outcome depends on the surgeon's ability to remove the entire epileptogenic zone, which may overlap with important functional areas. 4 As a result, there is a strong need for precise cortical functional mapping to minimize functional deficits while eliminating seizure onset zone.…”

“…In contrast, nonpharmacological management of epilepsy sometimes has unexpected effects. These treatments can be divided into several categories, including the ketogenic diet and its modifications, epilepsy surgery, neurostimulation (vagus nerve stimulation, deep brain stimulation, responsive cortical neurostimulation, and transcranial magnetic stimulation) [ 176 , 177 ].…”
