Background: From a historical perspective, many techniques of nipple reconstruction have been performed, including a graft from the contralateral nipple, composite grafts such as toe pulp or earlobe tissue and even an intra-dermal tattoo alone. This is the final stage of breast reconstruction, and is carried out only when the surgeon is confident that acceptable symmetry and shape of the reconstructed breast has been achieved. The technical challenges of nipple reconstruction include correcting position, maintaining adequate projection and creating an inconspicuous scar. An alternative to a surgically reconstructed nipple is the use of silicone prosthetic nipples. Materials and Methods: From August 2006 until September 2007, 80 cases of nipple/areola reconstruction were performed in our department (UDINE UNIV.) following mammary reconstruction or conservative breast surgery. Forty cases were carried out with the classical technique and another 40 cases with the introduction of our modification in the form of deepithelization of a semicircular area of the adjacent skin at the base of the flap. Postoperative follow-up as regards the nipple size, site, projection, symmetry and donnar scar were assessed. Patient satisfaction was also addressed and evaluated. Results: There were good to excellent results as regards nipple size, symmetry and projection. The technique is suitable for different autologous and implant reconstruction. The technique is an outpatient procedure, is easy and is not consuming time. Areolar graft from the contra-lateral areola is colouur matching and shows nearly no deference from the opposite one.Conclusions: Simple technique and not time consuming. Maintains the consistency and projection of the new nipple. Patient satisfaction. Minimal complication.

RESUMOA reconstrução do complexo areolo-papilar finaliza a reconstrução da mama após a mastectomia e melhora a satisfação das pacientes com o resultado final. Esta fase é crítica para acrescentar o realismo e a simetria da mama. Dentre as várias técnicas descritas, o uso de retalhos locais apresenta melhores resultados, apesar das desvantagens como, perda excessiva da projeção do mamilo e dificuldade técnica em mamas reconstituídas com implantes mamários, devido à pouca gordura subcutânea. O objetivo deste artigo é realizar uma revisão sobre as técnicas a disposição atualmente respeito à reconstrução do completo areolo-papilar. A reconstrução ideal deve proporcionar boa simetria, cor, textura e projeção, comparável com a mama normal 4 . Esta fase é crítica para acrescentar o realismo e a simetria da mama 3 . O primeiro retalho local para reconstrução do mamilo foi descrito por Berson 5 em 1946 e para sua realização eram utilizados três retalhos triangulares de pele total.Em 2005, Farhadi et al. 6 publicaram uma atualização sobre a reconstrução do CAP. Nesse artigo, eles apontam quatro princípios básicos descritos para obter sucesso: 1. O tempo ótimo para reconstrução ocorre aproximadamente entre 3 e 4 meses após a reconstrução mamária (embora alguns autores proponham a reconstrução mamilar simultânea a da mama). 2. Em uma reconstrução unilateral, o mamilo contralateral deve servir como modelo, adaptando a posição às assimetrias da mama residual.

Međutim, plastični hirurzi tehniku umetničkog tetoviranja umnogome mogu da nauče od profesionalnog tattoo majstora, kako bi tetovaža izgledala što realističnije i bila pravo umetničko delo. Cilj rada: Pokazati važnost, mogućnost i uticaj umetnosti tetoviranja bradavice -areola kompleksa, kod žena posle rekonstrukcije dojke. Metoda: Analiza i pregled studija o rekonstrukciji bradavice -areola kompleksa i uticaja na psihosocijalni život žene. Rezultati: Pacijenti sa gubitkom bradavice i areole od raka, ekscizija, traume ili sa urođenim nedostatkom doživljavaju veliku psihološku traumu. U slučaju raka dojke, kompletna rekonstrukcija dojke, nakon mastektomije, predstavlja značajan psihološki faktor kod žena. Rekonstrukcija bradavice može da se uradi na svim vrstama rekonstruisane dojke, u bilo koje vreme, nakon završetka hirurške intervencije. Rekonstrukcija bradavice je najlakša, sa tehničkog aspekta, ali je najvažnija iz estetske perspektive i istinska je umetnost u medicini. Zaključak: Kako su bradavice u centralnom fokusu kada se gleda dojka, rekonstrukcija dojke bradavice -areola kompleksa važna je komponenta za završetak rekonstrukcije dojke. Takođe se pokazalo da je značajan uticaj psihološkog faktora kod žena, u smislu samopouzdanja, u odnosu na one bez bradavica -areola kompleksne rekonstrukcije. Neophodno je da hirurg ima umetničku slobodu u radu i ponekad pomoć tattoo umetnika. Ključne reči: umetnost, tetovaža, dojka, bradavica. Summary:Introduction. Tattooing the nipple and areola is a well-established step in aesthetic and reconstructive surgery for women who have had cancer. However, plastic surgeons technique art of tattooing can learn a lot from professional tattoo artists to tattoo look as realistic as possible and it was a work of art.Aim. Show the importance and impact the ability of art of tattooing nipple-areola complex in women after breast reconstruction. Method. Analysis and review of studies on the reconstruction of the nipple-areola complex and the impact on the psychosocial life of the woman. Results. Patients with loss of nipple and areola from cancer excision, trauma or congenital lack of experience great psychological trauma. In the case of breast cancer, the whole breast reconstruction following mastectomy can provide significant psychosocial benefits for women. Reconstruction of the nipple can be done on all types of breast reconstructed at any time after surgery. Reconstruction of the nipple is the easiest from a technical point of view, but the most important from the aesthetic perspective and the true art of medicine.
