Introduction: Asthma is a chronic reversible inflammation which is caused by bronchial hypereactivity due to exposure to allergen and hyperproduction of certain specific IgE. Neutrophil in respiratory tract and high level of IgE in the blood serum is known to play in complex mechanism in asthma development. The role of total IgE serum and neutrophil serum in asthma development is still not well known. Aim: This study aim is to assess the correlation between level of total IgE and neutrophil serum with degree of asthma control. Methods: This is a cross sectional study with 35 adult subjects with atophy asthma (age 22-61), all of participant reside in Manado and its surrounding area. Degree of asthma control is categorized as intermittent, mild persistent, moderate persistent and severe persistent based on criteria by GINA. Patient with malignancy, sepsis, lung tuberculosis, diabetes mellitus, pregnant, and late stage chronic kidney disease are excluded from this study. Result: In this study there is no significant correlation between total IgE serum level and degree of asthma control (p>0.05.) also there is no significant correlation between each total IgE level and each degree of asthma control. There is significant positive correlation between neutrophil count and degree of asthma control (r = 0.309; p<0.05). The higher neutrophil count is higher in mild persistent asthma in comparison with intermittent asthma is found to be significant (p<0.05). Conclusion:The neutrophil count in serum is related to degree of asthma control. Especially neutrophil count in mild persistent is higher than intermittent asthma. There is no correlation between total IgE level with degree of asthma control in adult subject with atopic asthma.

The hallmark pathological features of asthma include airway eosinophilic inflammation and structural changes (remodelling) which are associated with an irreversible loss in lung function that tracks from childhood to adulthood. In parallel with changes in function, pathological abnormalities occur early, during the pre-school years, are established by school age and subsequently remain (even though symptoms may remit for periods during adulthood). Given the equal importance of inflammation and remodelling in asthma pathogenesis, there is a significant disparity in studies undertaken to investigate the contribution of each. The majority focus on the role of inflammation, and although novel therapeutics such as those targeted against T-helper cell type 2 (Th2) mediators have arisen, it is apparent that targeting inflammation alone has not allowed disease modification. Therefore, unless airway remodelling is addressed for future therapeutic strategies, it is unlikely that we will progress towards a cure for asthma. Having acknowledged these limitations, the focus of this review is to highlight the gaps in our current knowledge about the mechanisms underlying airway remodelling, the relationships between remodelling, inflammation and function, remodelling and clinical phenotypes, and the importance of utilising innovative and realistic pre-clinical models to uncover effective, disease-modifying therapeutic strategies. @ERSpublications We discuss mechanisms of airway inflammation and remodelling in asthma to uncover effective therapeutic strategies http://ow.ly/SsXiO

Резюме. Целью исследования явилось изучение взаимосвязи фенотипа и метаболизма нейтрофильных гра-нулоцитов у больных распространенным гнойным перитонитом (РГП) в динамике послеоперационного пе-риода. Обследовано 27 пациента с острыми хирургическими заболеваниями и травмами органов брюшной полости, осложнившимися РГП. Взятие крови производили перед операцией (дооперационный период), а также на 7, 14 и 24 сутки послеоперационного периода. В качестве контроля обследовано 67 относительно здоровых людей. Исследование фенотипа нейтрофильных гранулоцитов крови проводили методом проточ-ной цитометрии с использованием прямой иммунофлуоресценции цельной периферической крови. По сред-ней интенсивности флуоресценции оценивались уровни экспрессии поверхностных рецепторов. Активность НАД-и НАДФ-зависимых дегидрогеназ в нейтрофилах крови исследовали с помощью биолюминесцентного метода. Обнаружено, что у больных РГП уже в дооперационном периоде в периферической крови повышено со-держание CD62L + -, HLA-DR + -и CD64 + -нейтрофилов. Высокий уровень содержания CD62L + -клеток сохраня-ется в течение 24 суток послеоперационного периода, тогда как количество HLA-DR + -и CD64 + -нейтрофилов на 24 сутки послеоперационного периода снижается до уровня нормы. Динамике изменения содержания CD64 + -клеток в периферической крови больных РГП соответствует уровень экспрессии CD64-рецептора на мембране нейтрофильных гранулоцитов. Метаболизм нейтрофилов крови у больных РГП в до-и послеопе-рационном периоде характеризуется высокой интенсивностью субстратного потока по циклу трикарбоновых кислот, низкой активностью НАДФ-зависимой глутаматдегидрогеназы и аэробной реакции лактатдегидро-геназы. В дооперационном периоде и в течение 14 дней послеоперационного периода в нейтрофильных грану-лоцитах больных выявляется высокая активность анаэробной реакции лактатдегидрогеназы, характеризую-
