StreszczenieWstęp: Leczeniem przyczynowym chorych z nużliwością mięś-niową (ang. myasthenia gravis -MG) jest z wyboru usunięcie grasicy. U pacjentów z MG zastosowanie środków zwiotczających w celu przeprowadzenia operacji stanowi zagrożenie przedłużonej intubacji i wentylacji mechanicznej w okresie pooperacyjnym. Największym odkryciem ostatnich lat w anestezjologii jest środek odwracający blokadę nerwowo-mięśniową o nazwie sugammadeks. Preparat jest modyfikowaną gamma-cyklodekstryną, która opłaszcza i inaktywuje cząsteczki środków zwiotczających, tworząc z nimi kompleksy wydalane następnie przez nerki. Celem pracy była ocena użyteczności sugammadeksu w odwróceniu blokady nerwowo-mięśniowej wywołanej rokuronium po operacjach usunięcia grasicy u pacjentów z MG. Materiał i metody: Dwudziestu dwóch chorych z MG poddanych małoinwazyjnej operacji usunięcia grasicy podzielono na 2 grupy w zależności od użytego środka odwracającego zwiotczenie mięśni poprzecznie prążkowanych (S-sugammadeks vs. P-polstygmina). Monitorowanie głębokości blokady nerwowo-mięśniowej umożliwiło prawidłowe dawkowanie zarówno rokuronium, jak i środków odwracających blokadę nerwowo-mięśniową. Ocenie poddano czas od podania rokuronium do uzyskania blokady (T0), od podania środka odwracającego blokadę do powrotu wydolnego oddechu i ekstubacji (T1) oraz czas trwania zabiegu (T2). Oceniano wartości ciśnień tętniczych, skurczów serca i pulsoksymetrii w czterech punktach czasowych. Protokół znieczulenia w obu grupach zawierał indukcję i podtrzymanie znieczulenia metodą anestezji całkowicie dożylnej; analgezję zapewniono metodą zewnątrzoponową w odcinku piersiowym, z użyciem bupiwakainy.

Myasthenia gravis is a disease of great challenge to the anesthesiologist, because it affects the neuromuscular junction. Anesthetic management involves either muscle relaxant or non-muscle relaxant techniques. This case report documents the safe use of fentanyl, propofol and sevoflurane combination guided by bispectral index, without the use of muscle relaxants in a patient with myasthenia gravis who presented for meningioma surgery.

ÖZET GİRİŞMiyastenia gravis (MG) 1/20-40000 sıklıkta görülen, istemli kasların yineleyen aktivitesi ile artan, dinlenmekle ve antikolinesterazlarla tamamen ya da kısmen düzelen anormal kas yorgunluğu ile karakterize, otoimmün kökenli nöromusküler bir hastalıktır (1) . Bu hastalık Ig G antikorlarının neden olduğu nikotinik asetil kolin reseptörlerine karşı nöromusküler sistemde parsiyel ya da destüriktif bir bozukluktur.En önemli klinik bulgu iskelet kaslarında zayıflık ve yorgunluktur. Hastalık çoğu zaman oküler belirtilerle, en sık olarak da pitozis ile başlar ve buna diplopi eşlik eder. Miyastenia gravisli hastaların % 10'unda timoma, % 70 kadarında timus hiperplazisi bulunur. Bu tür hastaların büyük çoğunluğu timektomiden yarar gö-rürler (2) . MG tedavisine genellikle anti-kolinesteraz ilaçlarla başlanır, fakat orta şiddetli ve ciddi olgularda steroid ve timektomi tedavisi uygulanır. Konservatif tedavi başarısız olursa immunsupresif ilaçlar ve plazmaferez uygulanır (3) .Miyastenia gravisin anestezik yönetimi zordur. Olabildiğince rejyonel ve lokal anestezi tercih edilmelidir, ancak birçok cerrahi işlem için nöromusküler blokaj gerekli olmaktadır. MG hastalarında; tedavi için kullanılan antikolinesteraz ilaçlar ve hastalığın patogenezi nedeniyle kas gevşeticilere karşı değişmiş yanıt anestezi yönetimindeki zorluğun nedenini oluş-turmaktadır. MG hastalarında süksinilkoline rezistans ve nondepolarizan kas gevşeticilere karşı artmış duyarlılık mevcuttur (4) . MG hastalarının nondepolarizan kas gevşeticilere karşı duyarlılıkta büyük değişkenlik göstermesi nedeniyle ek dozlar titre edilerek dikkatlice kullanılmalıdır. Nöromusküler iletimin anesteziden çıkmadan önce nöromüsküler bloğun tam olarak tersine çevrilmesini sağlamak amacı ile nöromuskü-ler ileti monitörizasyonu sağlayan cihazlar tarafından sürekli izlenmesi gerekir (5) . Bu sayede nöromüsküler blokta uzama ve rezidüel bloktan kaçınılarak mekanik ventilatör gereksiniminde azalma sağlanabilir. Abstract: During sevoflurane anaesthesia, concentration-dependent inhibition of neuromuscular transmission was observed in MG and control patients. The inhibitory effects of sevoflurane were more prominent in MG patients with baseline T4/T1 <0.90.
