Neonatal Morbidity After Elective Repeat Cesarean Section and Trial of Labor

Abstract: Infants born by ERCS are at increased risk for developing respiratory problems compared with those born by TOL. However, TOL is associated with increased rates of suspected and proven sepsis. This appears to be limited to infants delivered by cesarean section after a failed TOL.

“…Most of the women in our study answered 'Neutral' regarding the maternal and fetal risks and benefits. It is possible that patients do not understand the issues involved [4,6,7,8,9,20,38,39,40,41,42,43]. It was of concern that Gamble et al found that women who preferred a cesarean section were poorly informed regarding the risks and overestimated the safety of the procedure [22].…”

“…23 Both the American Academy of Pediatrics (AAP) and the American College of Obstetricians and Gynecologists have cautioned against the prediction of later neurologic dysfunction solely on the basis of low Apgar scores at 5 minutes, as these have greater prognostic value when combined with peripartum complications, fetal acidemia, and signs of neonatal encephalopathy. 24 Nevertheless, 4 cohort studies 10,11,15,18 reported no significant difference in 5-minute Apgar scores between TOLAC and ERCD.…”

“…Neonatal mortality is defined as death in the first 28 days of age. 11,13 For this outcome, 6 cohort studies 8,11,12,[14][15][16] were identified that met study criteria established in the NIH systematic evidence review. We are aware of a large, population-based, birth certificate study by Menacker and colleagues 17 published after the review, but this study did not meet inclusion criteria, as there was no description of whether or how infants with congenital anomalies were excluded from the study population.…”
