НЕВІДКЛАДНИХ СТАНІВ Scientific Review Науковий оглядСтаття є частиною дисертаційної роботи на здобуття вченого ступеня доктора медичних наук «Нейроресусцитація та нейропротекція при тяжких перинатальних гіпоксично-ішемічних ураженнях головного мозку у доношених новонароджених». Шифр НДР (ДКР) ІН.03.11. Гіпоксично-ішемічна енцефалопатія (ГІЕ), попри значні досягнення щодо її діагностики й розуміння фетальної і неонатальної патології, залиша-ється одною з найбільш частих причин виникнення дитячого церебрального паралічу та інших видів тяжкого неврологічного дефіциту в дітей [1]. У Сполучених Штатах Америки й більшості технологічно розвинутих країн світу частота ГІЕ сягає, за даними різних авторів, від 1,5-4 до 1-8 випадків на 1000 пологів [2]. Захворюваність на ГІЕ є більш високою в країнах з обмеженими ресурсами, проте точних статистичних даних не існує. Фактично дані щодо нео-УДК 616-053.36:616.831:612.15]-071-073.432.19 Гіпоксично-ішемічна енцефалопатія в доношених новонароджених: сучасний стан проблеми Резюме. У статті поданий огляд літератури щодо гіпоксично-ішемічної енцефалопатії (ГІЕ), яка розвивається у немовлят після перенесеної перинатальної ішемії. Вона залишається істотною проблемою інтенсивної терапії в новонароджених у зв'язку з високим рівнем смертності й неврологічної інвалідності з дитинства, незважаючи на досягнуті певні успіхи щодо діагностики, моніторингу й методів лікування. Головною метою терапії треба вважати забезпечення церебральної перфузії й запобігання вторинним пошкодженням нейронів, у тому числі розвитку апоптозу. Серед методів дослідження й моніторингу стану центральної нервової системи у немовлят провідну роль відіграють: нейросонографія з допплерівським визначенням патернів мозкового кровотоку, моніторинг NIRS у поєднанні з амплітудно-інтегрованою електроенцефалографією. На сьогодні єдиним методом інтенсивної терапії з доведеною нейропротективною дією залишається терапевтична гіпотермія 33-35 °С протягом 72 годин. Її ефективність поки що залишається недостатньою: вона знижує частку поєднаного результату «смерть/тяжке порушення неврологічного розвитку» з 65 до 40-50 %, тому тривають активні пошуки методів лікування, які б могли покращити наслідки помірної й тяжкої ГІЕ. Щодо респіраторної підтримки перспективним виглядає застосування Neurally Adjusted Ventilatory Assist. До сьогодні не вирішене питання про ідеальний розчину для відновлення дефіциту об'єму циркулюючої крові в новонароджених. Кристалоїди через низький волемічний коефіцієнт мають нестійкий вплив на серцевий викид. Використання альбуміну обмежене через порушене проникнення гематоенцефалічного бар'єра при ГІЕ. Розглянута можливість використання 6% гідроксіетилкрохмалю 130/0,42, оскільки останні дослідження не продемонстрували додаткових ризиків порівняно із застосуванням кристалоїдів. Основним препаратом кардіотропної дії поки що вважається допамін, проте декілька робіт доводять переваги добутаміну щодо його впливу на системну й церебральну гемодинаміку. Lack of Progression of Intraventricular Hemorrhage in Premature Infants: Implications for Head Ultrasound Screening

Background: Premature infants are known to be at increased risk for intraventricular hemorrhage (IVH) in the first week of life. IVH may be “mild” (grade I or II) or “severe” (grade III or IV). A classification of mild is less frequently associated with later morbidity. Severe grade IVH may be associated with death or severe neurodevelopmental disability. Mild IVH is generally considered a static, non-progressive disease. Thus, infants that do not present with IVH or who present with mild IVH are unlikely to advance to severe IVH. Consequently, after initial imaging demonstrates a normal result, subsequent head ultrasounds (HUS) may be unnecessary.Methods: This is a retrospective, single-center study. We identified all preterm infants with birth gestational age </= 32 0/7 weeks admitted to the University of Massachusetts Memorial Medical Center Neonatal Intensive Care Unit between January 1, 2011 and December 31, 2016 who received a head ultrasound (HUS) during hospitalization. Each individual ultrasound was classified according to the attending radiologist’s documentation. Grades of IVH were defined per the Papile classification. Initial HUS was defined as HUS performed on day of life 3-10. Every subsequent HUS throughout hospitalization was read and recorded.Results: We identified 682 eligible preterm infants. Of these, 88 were excluded for lack of HUS data, 237 had initial HUS out of inclusion timing window (day of life 3-10), and 4 were excluded for other conditions associated with intraventricular hemorrhage, leaving 353 infants for analysis. Initial findings of severe IVH were relatively rare in this cohort. Of the 343 (97%) infants who had mild IVH (grade II or less) at time of initial screening, only 4 (1.2%) progressed to severe (grade III or IV). Each of these infants required mechanical ventilation for at least 40 days.Conclusions: Based on the results of this analysis, premature infants who have a normal (no IVH) HUS or mild IVH (grade I or II) on initial routine screening HUS without other risk factors may not require follow-up HUS. Infants with prolonged mechanical ventilation may require further screening despite reassuring initial HUS findings.
