Neoadjuvant hormonal therapy and external-beam radiotherapy versus external-beam irradiation alone for prostate cancer

Abstract: With the exception of sexual function (additional adjuvant HT and application of LHRH analog independently adverse), short-term NHT was not found to decrease quality of life after EBRT for prostate cancer.

“…FD: fraction dose; SV: seminal vesicles; TD: total dose. PTV and prostate volume [28]. NHT was not found to significantly decrease urinary or bowel QoL in contrast to several other studies that report increased urinary and bowel toxicity after EBRT and NHT [5,8,14,19,27,30,31].…”

“…The multivariate analysis in the present study demonstrated the independent effect of PS in the CTV for improved QoL in comparison to P. In spite of the well-known effect of NHT to decrease the prostate volume [21], NHT increased the frequency of symptoms after EBRT. We observed detrimental effects for patients with smaller prostates that we did not observe when patients with similar prostate volumes were compared in the matched-pair analysis [28].…”

“…Since prostate cancer is often a slowly growing disease causing little or no symptoms, treatment toxicity and quality of life (QoL) are of major concern [5,7,8,10,13,23,24]. QoL might be directly influenced by the anticancer treatment or indirectly via the onset of treatment-related side effects.…”

“…On the other hand, gynecomastia is not a negligible or merely cosmetic side effect, as it may lead a substantial number of patients (16%) to withdraw from their hormonal treatment [26] at the expense of oncological safety. The psychological impact of gynecomastia on the physical appearance should, therefore, not be underestimated [27] especially as increasing attention has recently been turned on quality of life aspects in the treatment of prostate cancer [10,20].…”
