Sexual dysfunction is a potential side effect of antidepressant drugs: this article presents a critical review of the current literature. Although many studies have been published on this subject, only some have used a validated sexual function rating scale and most lacked either a baseline or placebo control or both. In addition, many of the studies on sexual dysfunction associated with antidepressants are limited by other methodological flaws. However, there is consistent evidence to suggest that antidepressant medication adversely affects one or more of the 3 phases of sexual response (desire, arousal and orgasm). Antidepressants with strong serotonergic properties have the highest rate of sexual side effects. Clinicians must be aware of drug-induced sexual dysfunction, since its presence can have important consequences on clinical management and compliance.

Adequate sexual expression is an essential part of human relationships, enhancing quality of life and providing a sense of physical, psychological and social well-being. Unfortunately, depression is associated with impairments of sexual function and satisfaction. These problems can worsen a quality of life that is already reduced by the effects of depressive illness. The existing antidepressant drugs are far from ideal, most having adverse effects on sexual function. Unfortunately, the exact incidence of sexual dysfunction during treatment with many antidepressants is not known. Disturbances of sexual interest and performance will only be detected in a reliable fashion when systematic enquiries are made during the course of the standard clinical interview. Growing awareness of the adverse effects of many antidepressants on sexual function has led to some re-evaluation of the earlier claims for the good tolerability of many of the newer drugs. There is a clear need for further well-designed controlled studies of the effects of antidepressants on sexual function, so that this aspect of the tolerability of differing drugs can be assessed more reliably. “…First case was a 40-year-old man taking atomoxetine, second one a 25-year-old man using methylphenidate and third case was a 60-year-old man whom was started venlafaxine In addition, two cases were reported with a specific noradrenergic reuptake inhibitor reboxetine [4,11]. nefazodone, milnacipran and zotepine are other noradrenergic drugs that caused spontaneous ejaculation [12][13][14]. In our case, although venlafaxine is a dual reuptake inhibitor of serotonin and noradrenaline, thus we think potential mechanism related with spontaneous ejaculation is serotonergic mechanism, because patient was taking venlafaxine 75 mg per/day.…”

157Spontaneous ejaculation is a rare side effect of antidepressants. SSRIs, SNRIs, and reboxetine are reported to cause spontaneous ejaculation. Previous reports suggest to stop or change medication in the presence of this side effect, and less is known how to manage patients who want to continue the medication with this side effect. Mirtazapine is a noradrenergic and specific serotonergic antidepressant, mostly used for the treatment of depression. It is recommended as an add-on pharmacotherapy to relieve sexual side effects. Here, we demonstrate a case of spontaneous ejaculation, caused by venlafaxine, and reverted by mirtazapine.
