[3][4][5] NXG is usually associated with paraproteinemia; less often bone-marrow examination shows frank multiple myeloma. 6 Pathogenetically, several concepts have been considered paraproteins to cause NXG: (1) paraproteins bind to lipoprotein receptors and thereby stimulate/cause formation of NXG; (2) paraproteins bind lipoproteins, which are then ingested by macrophages via the scavenger low-density lipoprotein receptor pathway; and (3) NXG is analogous to scleromyxedema in its relationship to paraproteinemia, only with a different tissue reaction.…”
