Natural killer cell-derived large granular lymphocyte lymphoma of lung developed in a patient with hypersensitivity to mosquito bites and reactivated Epstein-Barr virus infection

Abstract: A 17-year-old female developed natural killer (NK) cell-derived large granular lymphocyte (LGL) lymphoma of the lung. She had a past history of hypersensitivity to mosquito bites (HMB). After an eight-year chronic, active Epstein-Barr virus (EBV) infection, she developed multiple lung lesions and pleural effusion. In the effusion, 60% of the cells were LGL. They were CD2+, 3-, 16+, 56+, 57+, 45RO+/RA + weak, and possessed strong NK activity. No rearrangement of T-cell-receptor genes was detected. From all thes… Show more

“…[24][25][26][27][28] A significant proportion of these patients die of Epstein-Barr virus-positive NK/T-cell lymphoma or hemophagocytic syndrome. [21][22][23]29 A comparable scenario can be proposed regarding the possible role of insect bites as a triggering factor in the presentation of ALCL. Insect bite-associated antigens could result in an influx of CD4 + T lymphocytes, some of them bearing the t(2;5) translocation.…”

“…The development of NK/T-cell lymphoma in patients with chronic active EBV infection is well-documented. A history of mosquito allergy was mentioned in patients with NK/T-cell lymphoma and leukemia [19,20] and was suggested to be pathognomonic for the development of these diseases. In the present series, only one patient with an EBV-positive nonnasal NK/T-cell lymphoma had a history of mosquito allergy.…”

“…The close association between HMB and NK cell proliferative disorders/ lymphomas has been known [8,9]. The affinity of EBVinfected lymphoid cells to blood vessels has been previously reported in EBV-positive NK/T-cell lymphomas [8,10,11]. The term angiocentric or angiodestructive pattern is often used to describe these lesions of the lymphoma tissue.…”
