Narcolepsy: Signs, Symptoms, Differential Diagnosis, and Management

Abstract: Narcolepsy is a chronic neurologic disorder characterized by excessive daytime sleepiness and cataplexy and less often by hypnagogic hallucinations and sleep paralysis. While patients report excessive daytime sleepiness and cataplexy as the more frequent symptoms of this condition, excessive daytime sleepiness is generally believed to be the most debilitating. Narcolepsy often is undiagnosed or misdiagnosed for a variety of reasons. Although confirmation of an initial diagnosis requires monitoring of physiolog… Show more

“…Although the phenomenon occurs in narcolepsy [Green and Stillman, 1998], isolated SP episodes are common in the general population with lifetime prevalence rates ranging from 5% to 62% [Dahlitz and Parkes, 1993]. Most studies, however, indicate that about 30% of individuals will experience at least one episode during their lives and about 5% will have one accompanied by the full range of visual, auditory, and tactile hallucinations [e.g., Cheyne et al, 1999].…”

“…Modafinil is a vigilance-enhancing compound approved as treatment for excessive daytime sleepiness (EDS) in narcolepsy (Bastuji and Jouvet 1988;Green and Stillman 1998). Modafinil is also effective in many other neurological diseases associated with EDS (Happe 2003;Rammohan et al 2002) including reduction of fatigue in multiple sclerosis (Rammohan et al 2002) and Parkinson's disease (Adler et al 2003;Happe et al 2001;Nieves and Lang 2002).…”
