Mycobacterium tuberculosis-Specific T Cell Functional, Memory, and Activation Profiles in QuantiFERON-Reverters Are Consistent With Controlled Infection

Abstract: Reversion of immune sensitization tests for Mycobacterium tuberculosis (M.tb) infection, such as interferon-gamma release assays or tuberculin skin test, has been reported in multiple studies. We hypothesized that QuantiFERON-TB Gold (QFT) reversion is associated with a decline of M.tb-specific functional T cell responses, and a distinct pattern of T cell and innate responses compared to persistent QFT+ and QFT- individuals. We compared groups of healthy adolescents (n=~30 each), defined by four, 6-monthly QFT… Show more

“…Serum activin A levels were measured in Gambian TB patients at recruitment and 6 months post-treatment, as well as in healthy tuberculin-skin test negative (TST - ) and TST + contacts, the latter being a heterogeneous population which may include people who are infected but healthy (who may progress to disease), as well as those who have cleared infection ( 29 , 30 ). Full details of the Gambian TB cohort are in Table 1 .…”

“…Although Mtb infection is thought to last lifelong, so-called reverters have been described, i.e. individuals who reverted from IGRA + to IGRA - remaining negative over long periods of time ( 133 , 135 ). The underlying mechanisms remain elusive and false IGRA - due to desensitization cannot be excluded.…”

“…In contact tracing studies, part of these conversions may be due to a de novo infection. Likewise, some of the reversions may be consequence of clearing the infection [ 2 , 3 ]. However, since a significant proportion of them occur with values near to the threshold of positivity of the test–uncertainty zone- suggests that in many instances they are due to biological variability of the technique.…”
