Mycobacterium chelonae–Mycobacterium abscessus complex clear corneal wound infection with recurrent hypopyon and perforation after phacoemulsification and intraocular lens implantation

Abstract: We report a clear corneal wound infection occurring in a 74-year-old man caused by a member of the Mycobacterium chelonae-Mycobacterium abscessus complex, presenting as crystalline keratopathy with recurrent hypopyon. This led to perforation after phacoemulsification with posterior chamber intraocular lens implantation. Only after corneal biopsy of the incision was the causative organism isolated and found to be sensitive to clarithromycin and ciprofloxacin. Despite aggressive therapy, a full-thickness corneal… Show more

“…Moreover, orbital infection has been reported after penetrating injury to the orbit, and after blepharoplasty (40, 41), certainly caused by an exogenous contamination. Several cases of endophthalmitis have been described following eye surgery, intravitreal injections or lens implantations, all procedures probably having been done under non‐sterile conditions (42–46).…”

“…Tuberculosis is considered one of the great masqueraders; because the purified protein derivative test demonstrated 15 mm of erythema although TB gold was not available for her at that point, the patient was started on antituberculosis drugs, including ethambutol, rifampin and isoniazid. Infection by Mycobacterium has been reported after laser‐assisted in situ keratomileusis, extracapsular cataract surgery, endokeratoplasty and intravitreal injection 8,9 . There is only one reported case of Mycobacterium chelonae – Mycobacterium abscessus complex corneal wound infection after phacoemulsification and intraocular lens implantation 8 .…”

“…Various mycobacterium species, including Mycobacterium goodii, 1 M. abscessus, 2 and M. chelonae, 3 have been reported, but to the best of our knowledge, M. tuberculosis has not been reported. We report a case of endophthalmitis due to M. tuberculosis (MTB).…”
