Music therapy for early cognitive rehabilitation post-childhood TBI: An intrinsic mixed methods case study

Abstract: The process of micro-analysis illuminated four discrete participant behaviours not apparent in the immediate clinical setting. The results of this case suggest that the use of familiar song as a music therapy intervention may harness early patient responsiveness to foster cognitive rehabilitation in the early acute phase post-TBI.

“…It has shown positive results in reducing the levels of anxiety, depression, agitation and in inducing stable mood state. Enhanced adaptive behavior following music-based intervention has been observed even during recovery from coma and later in both adult and children population (3339). Music-based intervention has led to improvement in speech production and sensory perceptions (40, 41).…”

“…Music-based intervention has led to improvement in speech production and sensory perceptions (40, 41). Studies suggest that use of musical intervention facilitates early responsiveness in patients which in turn foster cognitive rehabilitation in the early acute phase following TBI (39). The more recent frontier is music-based CR in various neurological and neurosurgical conditions (33, 35).…”

“…Haslbeck's rich articulation of the musicality itself is not only immensely useful to the music therapy clinician but also promotes a rigour which is key to replication of contingent musicality in shared experiences of live music. Though previous studies have employed similar tri-tiered analyses to elucidate the therapeutic relationship in contingent singing (Bower, Catroppa, Grocke, & Shoemark, 2013;Shoemark & Grocke, 2010), Haslbeck contributes the vital musical structuring of that relationship. Micro-analysis allows for the comprehensive mapping of musical and therapeutic alliances which will provide a new benchmark for clinicians to demonstrate the powerful "feeling" of music therapy.…”
