Muscle involvement in juvenile idiopathic arthritis

Abstract: Changes in leg muscle biopsies appear to be common in children and teenagers with JIA. The presence of inflammatory cells in the muscle and expression of MHC class II on muscle fibres may be a sign of inflammatory myopathy. There are no findings of type II muscle fibre hypotrophy or neuropathy, as in adults with RA.

“…Compared to OA at the hip and knee, ankle OA is rare 49 and, therefore, it was hard to find patients that fit the inclusion criteria. Although muscle and EMG physiological studies usually deal with small subject numbers, 8,16,37,40,50,51 this study found a series of significant results. Another limitation is the comparison of EMG results across muscles and subjects.…”

“…6,8,[15][16][17][18][19][20][21] It has been shown that in knee OA the quadriceps strength is usually reduced by 14% to 45% compared to control subjects. 6,8,18,19,21 Sirca et al 22 reported that degenerative hip OA was associated with an atrophy of type II fibers.…”

“…In addition, pain from increased intra-articular pressure can cause muscle inhibition and weakness. 10,11 Once joint inflammation subsides, the main limitation to repair of synovial tissue is the early establishment of fibrous adhesions with resultant connective tissue shortening and soft tissue contractures, which further accentuate pain and muscle inhibition reducing joint mobility. [12][13][14] These pathophysiological changes often lead to poor posture, body alignment and growth disturbance with reduced weight bearing, bone demineralisation and adapted gait patterns.…”

“…U dziecka dominowały wtedy stany gorączkowe i objawy związane z zapaleniem mięśni. Jakkolwiek bóle mięśniowe należą do typowego obrazu klinicznego uMIZS, to obiektywne objawy zapalenia mięśni (patologiczny zapis EMG, podwyższona aktywność enzymów mięśniowych) wyjątkowo rzadko obserwuje się w przebiegu tej choroby [5][6][7][8]. Z tego względu podejrzenie zapalenia wielomięśniowego na podstawie kryteriów diagnostycznych tej choroby w tym okresie choroby było uzasadnione (tab.…”
