Background: Few studies have documented the outcome of conservative treatment of hallux valgus deformities on pain and muscle strength. We sought to determine the effects of foot mobilization and exercise, combined with a toe separator, on symptomatic moderate hallux valgus in female patients. Methods: As part of the randomized clinical trial, 56 women with moderate hallux valgus were randomly assigned to receive 36 sessions for 3 months or no intervention (waiting list). All patients in the treatment group had been treated with foot joint mobilization, strengthening exercises for hallux plantarflexion and abduction, toe grip strength, stretching for ankle dorsiflexion, plus use of a toe separator. Outcome measures were pain and American Orthopedic Foot and Ankle Society (AOFAS) scores. Objective measurements included ankle range of motion, plantarflexion and abduction strength, toe grip strength, and radiographic angular measurements. Outcome measures were assessed by comparing pretreatment, posttreatment, and 1-year follow-up after the intervention. Mixed-model analyses of variance were used for statistical assessment. Results: Patients who were treated with 3 months of foot mobilization and exercise combined with a toe separator experienced greater improvement in pain, AOFAS scores, ankle range of motion, hallux plantarflexion and abduction strength, toe grip strength, and radiographic angular measurements than those who did not receive an intervention 3 months and 1 year postintervention (P < .001 for all comparisons). Conclusions: These results support the use of a multifaceted conservative intervention to treat moderate hallux valgus, although more research is needed to study which aspects of the intervention were most effective.

RESUMEN:El desempeño de los pies influye en la postura, marcha y en el uso de calzados. Deformidades pueden surgir debido a alteraciones estructurales provocadas por el desequilibrio muscular como parece suceder en el hallux valgus (HV). El Músculo aductor del hallux (MAH) es considerado importante en la presencia de ese desequilibrio. Analisamos la acción del MAH, en la dinámica del hallux y del antepie sano, así como en la génesis del HV. Se disecaron 28 pies aislados (3 com HV), fijados en formol, del Laboratorio de Anatomía de la Facultad de Medicina de Petrópolis/FASE/RJ para obtener los datos morfométricos del MAH: largo de las cabezas (L-CT; L-CO); ancho de las cabezas (A-CT; A-CO) y ángulo entre las cabezas (q). Los valores medios (mm), obtenidos en los pies sin deformidades: L-CT= 27,04; L-CO= 51,45; A-CT=13,23; A-CO=17,58; q (CT/CO)=31°; y en los con HV: L-CT=23,80; A-CT=12,03; L-CO=48,25; A-CO=16,89; q (CT/CO)=52º. La morfología del MAH osciló entre pies sin y con HV. El MAH al fijarse en las articulaciones metatarsofalángicas (CT) y en el hueso sesamoideo lateral del hallux (CT+CO) puede ser causa o consecuencia de la variación del q (CT/CO), llevando al valgismo del hallux. La amputación del hallux podrá aplanar el arco transverso anterior debido a la tensión muscular, dificultando las funciones del pie en el apoyo y en la marcha.PALABRAS CLAVE: Músculo aductor del hallux; Hallux valgus; Anatomía del Pie; Deformidad del Pie. INTRODUCCIÓNEl pie, a diferencia de la mano, presenta entre los dedos y el talón una bóveda formada por los huesos del tarso, dispuestos inicialmente lado a lado. Sin embargo, el peso del cuerpo obliga la disposición de estos huesos en pirámi-de, caracterizando al llamado órgano "Pie" (Kahn, 1960).Alteraciones oseomioarticulares y vasculares pueden provocar trastornos de la biomecánica del pie y de la marcha (Haines & McDougall, 1954;Hoffmeyer et al., 1988;Canale, 2007). El hallux valgus (HV) es una deformidad frecuente cuyo desvío del hallux, en valgo (+ de 8° a la lateral), y del 1er metatarsiano, en varo (+ de 15° a la medial) causa el Síndrome de Insuficiencia del 1er dedo. El término "Juanete" -prominencia medial a nivel de la cabeza del 1er metatarsiano -acompaña el HV, pero no debe ser utilizado como sinonímia de la deformidad (Herbert et al. 2003;Viladot, 2003). El HV es complejo, estructuralmente inconstante, de diversos grados de gravedad, deformando la región medial del antepie, promoviendo alteraciones estéti-cas, biomecánicas y dolorosas, principalmente, en el uso de calzados.Hay relatos de alteraciones de la morfología muscular en el HV, siendo el músculo aductor hallux (MAH) considerado importante en la génesis de la deformidad: la contractura y aducción del 1er dedo presionando la cabeza del 1er metatarsiano, intensificando el valgismo del hallux (Vasconcellos, 1987;Fernández, 1991).El envolvimiento del MAH en el HV y la relevancia de esa deformidad, son los objetivos de este estudio.
