Multivisceral resections in colorectal cancer

Abstract: From 1969 to 1983 a total of 1918 patients with colorectal cancer were treated by curative resection. One hundred twenty one patients in this group had multivisceral organ involvement, necessitating extended multivisceral radical resection. Tumor infiltration was proven histologically in 55 percent, while 45 percent had inflammatory adherence to the attached organ only. Postoperative mortality was 12 percent. Dukes' A and B stages were present in 57 percent. The five-year survival rate (postoperative mortality… Show more

“…Carcinoma involving adjacent organs was found in 43 (9.1%) of our 473 patients. An incidence of 5.5 to 12.0% has been seen in other studies2, 3,[8][9][10][11] There was little clinicopathological difference between the patients with and without colorectal cancer involving adjacent organs, with the exception of tumor size which was larger in patients with a carcinoma involving adjacent organs.…”

“…It is reported that the local recurrence rate was higher when adherent organs were dissected from the tumor than in cases in whom en bloc resection was performed (69 vs. 18%, respectively) (9). And it is also reported that the five-year survival rate was 17% after inadvertent dissection or rupture of the tumor, compared with 49% after en bloc resection (10). It is difficult to diagnose precisely whether or not an actual tumoral invasion exist during surgery, and therefore, en bloc multivisceral resection is needed to obtain clear resection margin (R0) according to these results.…”

“…Although we attempted TAE for the feeder artery, the patient developed multiple organ failure. Mortality rates have been reported to be higher in patients with multiple resections compared to those treated by conventional colorectal resection (12% vs 6%, respectively) [9] . With recent advances in surgical management, the operation risk associated with PD has been decreasing.…”
