Multistate utilization, processes, and outcomes of carotid endarterectomy

Abstract: The striking variation among states suggests that there is room for improvement in the utilization, care processes, and outcomes of CEA. All surgeons performing CEA should participate in outcome assessment and adopt protocols that include the routine administration of antiplatelet agents preoperatively, the use of heparin intraoperatively, and patch angioplasty of the endarterectomy site.

“…Unhappily, reports indicate that there are many strokes when the procedure is being done with a greater than acceptable operative risk. 16 • Reviews and publications of Medicare records by Kresowik 19 (a surgeon) and others indicate that in multiple states in the United States, operators are not performing CE in a safe manner. In these areas the operating room must be added as a risk factor for patients with asymptomatic carotid stenosis in controlled trial angioplasty and tenting has proven twice as risky so that it is not a substitute for the hazards of endarterectomy independent auditing is called for.…”
