Multistate Comparison of Cost, Trends, and Complications in Open Versus Endoscopic Carpal Tunnel Release

Abstract: Background: Surgical carpal tunnel release is performed by either open carpal tunnel release (OCTR) or endoscopic carpal tunnel release (ECTR). The purpose of this study was to assess differences in intraoperative and postoperative complications, trends, and costs between OCTR and ECTR. Methods: State Ambulatory Surgery and Services Databases (SASD) files for California, Florida, and New Jersey were queried for patients who underwent OCTR and ECTR between 2000 and 2014. Patient demographics, comorbidities, int… Show more

“…Third, the wrist and hand muscles are usually weak and usually get smaller due to severe compression of the median nerve. Lastly, doctors recommend surgery when the syndrome's symptoms last more than six months without any relief [26]. In essence, non-surgical therapies would be the best option when one suspects they have carpal tunnel syndrome.…”

“…Despite these limitations, the methodology used in this study has been demonstrated to be useful in several previous orthopaedic studies. 1,2,4,12,13,24 The goal of this study was to analyze how surgeon- and patient-specific factors are associated with cost, and further studies may aim to delve deeper into the trends identified. Although there are limitations inherent in this analysis, the trends identified can prove useful to surgeons and patients alike.…”

“…Other studies using different accounting methods all generally agree that the immediate procedural costs of ECTR exceed those of OCTR, some estimates by as much as $2,000 per case. 29,30 Direct costs are not limited to the choice of procedure, but also encompass surgical setting (procedure room versus operating room) and anesthesia choice (local, monitored anesthesia care, Bier block, and general). Kazmers et al used their institutional data to calculate total direct costs of carpal tunnel release surgical encounters and found that performing OCTR in the procedure room under local anesthesia was the least costly technique.…”
