“…Some authors have reported good results with core decompression. This surgical method has been recommended for rapid and complete reduction of symptoms, with a return to normal MRI patterns [2,22,25], and is based on the theory that pain in BME and avascular necrosis is caused by increased intramedullar pressure. Although this treatment has a low rate of complications and can be performed as an outpatient procedure, up to six weeks of partial or complete weight bearing are recommended, sometimes followed by physiotherapy.…”

We consider that THO should be renamed bone marrow oedema syndrome (BMOS), and that MRI should play an essential part as the diagnosis must still be established by exclusion.…”
