Moxifloxacin—Warfarin Interaction: A Series of Five Case Reports

Abstract: Healthcare professionals should consider moxifloxacin for the potential to interact with warfarin. Routine, frequent INR monitoring for patients previously stabilized on warfarin during initiation and discontinuation of moxifloxacin may help detect this potential interaction.

“…17 Several postmarketing case reports, however, have suggested a possible drug interaction. [10][11][12][13] In these reports, the authors described a total of 11 cases where most of the patients appeared to have a stable anticoagulation regimen and experienced rapid elevations in their INR values after the initiation of moxifloxacin. Although many of the supratherapeutic INRs were recorded several days after the start of daily moxifloxacin, an interaction and steep rise in INRs could be seen even after the first dose of moxifloxacin was administered in four of these cases, 10,11 similar to what was observed in our patient.…”

“…Several reports have documented such elevations and potential interactions. [5][6][7][8][9][10][11][12][13] An elevated INR is associated with an increased risk of bleeding. Major bleeding, such as intracranial hemorrhaging, is associated with significant morbidity and mortality.…”
