Preterm birth is a multi-problematic event and can greatly affect the mother psychological wellbeing. The aim of this study was to assess the relationship among depression, anxiety, and mother-infant bonding in mothers of premature babies. A descriptive correlational research design was utilized in this study. A convenient sample of sixty mothers was recruited in this study. The study was conducted at the Neonatal Intensive Care Unit (NICU), at El Manial University Hospital. Data for this study were collected using four tools including socio-demographic data sheet, Beck Depression Inventory, Beck Anxiety Inventory and Mother-Infant Bond Scale. The results of the current study showed that mothers with premature babies suffer from severe level of anxiety and have high negative attitude toward their babies. Also, the study revealed that there was no statistical significant relationship between mothers' depression, anxiety and their bond with their babies. However, there was statistically significant direct relationship between mothers' depression and anxiety. The study concluded that, having premature baby can affect the mother's psychological state. The study recommended that, mothers of premature babies are in great need for psychological support and guidance. It is crucial to follow-up those mothers during their premature babies' development.

ObjectivesThe principal aim was to investigate the feasibility of assessing mother-infant interactions at discharge and at 6 months infant corrected age in singletons born before 32 weeks of gestation. The secondary aims were to describe these interactions and their disorders, explore the association between maternal emotional state and the interactions, and assess the relationship between disordered interactions and infant social withdrawal behaviour.MethodsOLIMPE is an ancillary study of the population-based study EPIPAGE 2, which recruited preterm neonates in France in 2011. 163 dyads participated at discharge and 148 at 6 months. Interactions were observed with the Attachment During Stress (ADS) scale, which includes two behavioural subscales, for the mother (m-ADS) and her infant (i-ADS). Two professionals independently completed the ADS scales for one third of the observations. Maternal emotional state was assessed using self-administered questionnaires of depression, anxiety, and stress. Infant’s social withdrawal behaviour at 6 months was measured by the Alarm Distress Baby scale.ResultsAt discharge, 15.3% of the m-ADS scales and 43.3% of the i-ADS scales had at least one unobserved component. At 6 months, all items on both scales were noticeable in >90% of the dyads. Reliability, estimated by the kappa coefficient, ranged between 0.39 and 0.76 at discharge, and between 0.21 and 0.69 at 6 months. Disordered interactions were indicated on 48.6% of the m-ADS scales and 36.5% of the i-ADS scales at discharge. At 6 months, these rates were 32.6% and 26.0%. Disordered interactions at 6 months were associated with identified disorder at discharge. Insecure infant attachment was not influenced by maternal mental health but was strongly associated with infant social withdrawal behaviour.ConclusionsThe ADS scale can be used to screen for early interaction disorders after premature birth and may help to target dyads that would most benefit from early intervention.

Raktažodžiai: priešlaikinis gimdymas, psichinė būklė, nerimas, HAM-N. SantraukaDarbo tikslas: įvertinti prieš laiką pagimdžiusių moterų psichinę būklę ir nustatyti psichologo poreikį postnataliniu laikotarpiu. Metodika: 2016 m. lapkritį -2017 m. vasarį Vilniaus universiteto ligoninėje Santaros klinikose (VUL SK) Akušerijos ir ginekologijos centre atliktas prospektyvinis anoniminis anketinis tyrimas. Į tyrimą buvo įtraukta 51 prieš laiką pagimdžiusi moteris, jos buvo apklaustos pogimdyminėse palatose 1 -3 parą po gimdymo. Buvo vertinta gimdyvių psichinė būklė, atsižvelgiant į demografinius duomenis, akušerinę anamnezę bei šio nėštumo eigą. Psichiniam ir somatiniam nerimui vertinti naudota modifikuota, su gydytoju psichiatru bei gydytoju akušeriu-ginekologu aptarta Hamiltono nerimo skalė (HAM-N). Duomenys apdoroti SPSS 20 programa. Patikimumas apskaičiuotas naudojant chi kvadrato testą (p≤0,05). Rezultatai: 68,6 proc. (n=35) moterų patyrė emocinių sukrėtimų dėl priešlaikinio gimdymo. Didžiausią įtaką bendrai psichinei būklei perinataliniu laikotarpiu turėjo patirtas nerimas (60,8 proc., (n=31), baimė (13,7 proc., (n=7), nemiga (13,7 proc., (n=7) ir įtampa (11,8 proc., (n=6). Gimdyvių, vertintų pagal Hamiltono nerimo skalę, vidurkis buvo 10,45 balai (min -0, max -28 balai). Psichinio nerimo vidurkis -6,7 balai, somatinio -4,14. 76,5 proc. Išvados. Net 23,6 proc. tiriamųjų nustatytas vidutinio sunkumo ar stiprus nerimo sutrikimas. Tam tikri demografiniai rodikliai, akušerinės anamnezės veiksniai bei šio nėštumo eigos padariniai turi reikšmingos įtakos atskiriems psichinio nerimo bei somatinio nerimo simptomams. Net 17,6 proc. tiriamųjų norėtų, kad joms būtų suteikta psichologo konsultacija po gimdymo. ĮvadasPagal Pasaulio sveikatos organizacijos pateikiamą apibrėžimą (PSO) priešlaikiniu gimdymu vadinamas toks gimdymas, kuris įvyko nuo 22-os iki 37-os gestacijos savaitės pabaigos (36 sav. + 6 d.), arba praėjus mažiau nei 259 d. nuo pirmos paskutinių moters menstruacijų dienos (1,2). Jei tikslus nėštumo laikas nėra žinomas, laikoma, kad gimdymas yra priešlaikinis, jei naujagimis sveria ≥500 g (3).Nuo 1990 iki 2005 m. Europoje priešlaikinių gimdymų padaugėjo 22 proc. Šią tendenciją lemia keletas veiksnių: daugėja daugiavaisių nėštumų dėl pagalbinio apvaisinimo procedūrų, gimdo vis vyresnės moterys, kurios dažniau serga šalutinėmis ligomis, joms dažniau pasitaiko nėštumo komplikacijų (5).Priešlaikinis gimdymas yra dažniausia (75-80 proc.) perinatalinio mirtingumo ir sergamumo (įvairių neurologinių, regėjimo, klausos, motorinių ar psichinių problemų) priežastis visame pasaulyje, dėl kurios 2010 m. buvo prarasta 1,08 mln. gyvybių 193 šalyse (2,5).Pirmieji moksliniai tyrimai, siekiantys suprasti streso poveikį nėštumo eigai, prasidėjo 1970 m. (7). Skelbiama ne viena mokslinė publikacija, kuri įrodo, jog yra stiprus ryšys tarp nėštumo ir moters psichinės būklės.
