Morphological Stages of Pilomatricoma

Abstract: In 1983, Ackerman proposed that pilomatricoma represents an infundibular-matrix cyst in its early stages. To study the evolution of this cystic neoplasm, we examined 118 lesions from 116 patients with pilomatricoma histopathologically and categorized the lesions into four distinct and chronological stages: early, fully developed, early regressive, and late regressive. Early lesions (eight cases) were small cystic structures lined by squamoid and basaloid epithelium containing keratin filaments and faulty hair … Show more

“…Ultimately, the lesion eventuates as a so-called 'ancient' pilomatricoma with a florid foreign body reaction and abundant calcification in the adjacent stroma. 90,91 Metaplastic bone formation in these lesions may subsequently ensue. A perforating variant of the pilomatricoma is uncommonly seen, whereby the necrotic material and shadow cells are extruded through the epidermis or through a hair follicle.…”

“…In contrast, pilomatrix carcinoma shows broad geographic necrosis with an infiltrative margin and cytological alterations, including fully transformed malignant change. 90,91,[103][104][105][106][107][108][109][110][111][112][113][114][115] Matrical differentiation may only be focal in these lesions. Tumors have the potential to spread to regional lymph nodes and produce fatal disseminated metastatic disease.…”

“…Pilomatricoma, a skin adnexal tumor that recapitulates hair growth, [1][2][3] is by far the most common epithelial neoplasm of childhood. 4 The proliferating cells in pilomatricoma seem distinctly programmed to undergo terminal differentiation and death, which is perhaps responsible for the tumor's inevitably benign clinical course.…”
