BRANEMARK et colI. (1969) reported that all contrast media currently in use cause microcirculatory disturbances to a varying degree to the hamster's cheek pouch after their local application. Signs of damage to the endothelium were demonstrated by an increasing number of white cells adhering to the vessel wall and sometimes thrombus formation. The present investigation was therefore undertaken to find out whether these microcirculatory changes may be associated with an increase in vascular permeability. As all contrast media are hypertonic solutions special consideration was given to the possible relation between the osmolarity of the media and changes in the vascular permeability. The significance of the hypertonicity of the contrast media was also analysed in simultaneously performed vital microscopy investigations.Material and Methods. SUMMARY Metrizamide represents a significant advance in patient tolerance to ascending phlebography of the leg. The objection to its general introduction is its high cost. From a randomised, prospective, within patient study using metrizamide 280mg/ml for one leg and 180mg/ml for the other, it is concluded that a substantial reduction in cost can be made using the more dilute medium while maintaining full diagnostic quality.
