Monitoring Performance for Blood Pressure Management Among Patients With Diabetes Mellitus

Abstract: Background Performance measures that reward achieving blood pressure (BP) thresholds may contribute to overtreatment. We developed a tightly-linked clinical action measure designed to encourage appropriate medical management; and a marker of potential overtreatment, designed to monitor overly aggressive treatment in the face of low diastolic BP. Methods We conducted a retrospective cohort study in 879 Department of Veterans Affairs (VA) medical centers and smaller community based outpatient clinics (CBOCs). … Show more

“…For example, several recent studies have shown that implementation of underuse measures can unintentionally encourage overuse of care. [4][5][6][7] In contrast, overuse measures have the potential to discourage utilization when care is actually indicated, promoting underuse. In light of such concerns, Mathias and Baker have recommended that the potential unintended effects of overuse measures be carefully considered during measure development, and that efforts be taken to mitigate and monitor for such effects.…”

“…Measures that are clinically meaningful are essential to improving the appropriateness of care that we deliver. Too often performance measures have been blunt instruments that were not tailored to individual patient circumstances, sometimes leading to overuse of unnecessary or even harmful services 6 for some patients. One way to make measures more meaningful is to tailor them to the individual patient likelihood of benefit for the treatment or procedure being considered.…”

“…[27] This risk is becoming more acute given an aging population and an increase in the numbers of people with multi-morbidity who may require complex trade-offs in optimal single disease management to achieve individualized personcentered care. However, such case-mix adjustments are difficult to define.…”
