As a consequence of the massive restructuring of drug availability, heroin injection in Hungary was largely replaced by the injecting of new psychoactive substances (NPS) starting in 2010. In the following years in our sero-prevalence studies we documented higher levels of injecting paraphernalia sharing, daily injection-times, syringe reuse, and HCV prevalence among stimulant injectors, especially among NPS injectors. Despite the increasing demand, in 2012 the number of syringes distributed dropped by 35% due to austerity measures. Effects of drug market changes and the economic recession may have future epidemiological consequences. Study limitations are noted and future needed research is suggested.

IntroductionAbout one quarter of human immunodeficiency virus (HIV) infected persons in Serbia have also been found to be hepatitis C virus (HCV) co-infected. In the general population, HCV genotype 1 has been shown to be the most prevalent one. Here, we present the first study on the distribution of HCV genotypes among HIV/HCV co-infected patients in Serbia, in relation to epidemiological and clinical features.Material and methodsThe study included HIV/HCV co-infected and a group of HCV mono-infected patients in the period 1998–2012, with collection of epidemiological, clinical, and behavioral data using a standardized questionnaire. The HCV genotyping to the level of pure genotype was performed by reverse hybridization.ResultsIntravenous drug use (IDU) was found to be significantly more prevalent among the co-infected patients (p < 0.01). HCV genotype 1 was detected in 87% of patients with mono-infection, compared to 46.3% of patients with co-infection (p < 0.01); genotypes 3 and 4 were significantly more common among co-infected patients (6% and 5%, vs. 27% and 25%, respectively). Multivariate logistic regression confirmed IDU, infection with non-1 HCV genotype and HCV viral load over 5 log to be predictors of HIV co-infection.ConclusionsThe HCV genotypes 3 and 4 were found to be significantly more prevalent among HIV/HCV co-infected patients in Serbia, compared to HCV mono-infected patients, but also more prevalent compared to the European HIV/HCV co-infected cohort. History of IDU represents an independent predictor of HCV genotypes 3 and 4 infection, with important implications for treatment.

ÖzetAmaç: Hepatit C virus (HCV) enfeksiyonları, yüksek kronikleşme oranı, karaciğer yetmezliği, siroz ve hepatosellüler kanser oluşturması nedeniyle tüm dün-yada önem taşıyan bir sağlık sorunudur. Bu çalışmadaki amacımız; enzim immunoassay (ELISA) testi ile anti-HCV pozitifliği tespit edilen hasta grubunda, HCV RNA'nın araştırılmasının yanı sıra, hasta serumunda HCV RNA varlı-ğının, alaninaminotransferaz (ALT) düzeyleri ve HCV genotipleri ile ilişkisinin incelenmesidir. Gereç ve Yöntem: Laboratuvarımıza çeşitli servislerden gelen anti-HCV pozitif hastalarda, HCV RNA düzeyi ve genotip tayini kantitatif real time polimeraz zincir reaksiyonu (PCR) Montania 483 (Anatolia Geneworks HCV, Türkiye) ile üretici firma talimatları doğrultusunda yapılmış-tır. Anti-HCV testleri ELISA (Advia Centaur XP, Germany) yöntemiyle çalışıl-mış, ALT değerleri retrospektif olarak incelenmiştir (Olympus AU2700). Bulgular: Anti HCV'si pozitif, HCV RNA çalışılan toplam 338 hasta serumunun 123'ünde (%36.3) HCV RNA pozitif bulunmuştur. HCV RNA pozitif hastalar içinde ALT referans üst düzey 40 IU/L olarak kabul edildiğinde, bu değerin üzerinde olan 59 (%77.6) hasta tespit edilmiştir. HCV RNA pozitif hastalar ile ALT düzeyi 40'ın üzerinde olan hastalar arasındaki ilişki anlamlı bulunmuştur (p<0.001). HCV genotip dağılımına bakıldığında genotip 1b %84 (n=37), genotip 1a %11.4 (n=5), genotip 3 %2.3 (n=1) ve genotip 4 %2.3 (n=1) olarak tespit edilmiştir.Tartışma: Sonuç olarak, anti HCV pozitif olan hastalarda moleküler yöntemlerle HCV RNA bakılmalı, viral yük tespit edilmelidir. Çalışma-mızdaki genotip sonuçlarımız bulunduğumuz coğrafya ile uyumludur. HCV enfeksiyonlu hastaların tanı, tedavi ve takiplerinde HCV RNA tespitinin, HCV genotipi ve karaciğer enzim testleri de göz önünde bulundurularak bir bütün olarak değerlendirilmesi gerekmektedir.
