Molecular Characterization of Group B Streptococcus Serotypes By Multiplex Polymerase Chain Reaction

Abstract: OBJECTIVE: Group B Streptococcus (GBS) serotypes (Ia, Ib and II to IX) are classified based on variations in their capsular polysaccharide; their prevalence differs between different geographic areas. We examined the prevalence of all GBS serotypes in rectal and vaginal swab samples obtained from 363 pregnant women followed at a Brazilian referral center (Hospital da Mulher Professor Doutor José Aristodemo Pinotti); bacterial susceptibility to antibiotics was further determined. METHOD: Prevalence of positive… Show more

“…[32] Both studies were conducted in Rio de Janeiro. In São Paulo, serotype Ia was the most abundant followed by V and II in a study conducted in 2015-2016 [54], suggesting that the increased prevalence in serotype Ia may be related with a period of time. In Paraná, serotype Ia was also the most prevalent; however, there was a surprisingly high number of serotype IV strains [53].…”

“…There is a divergence in serotype distribution among pregnant women from different locations in Brazil [32,[51][52][53][54]. Few studies evaluated GBS serotypes, and from 2008 to 2017, most of them were performed in the Southeast Region (Fig.…”

“…However, there are concerns about resistance to antibiotics, as it may involve prophylaxis and inadequate treatment 25 . In studies conducted in Brazil, for example, resistance to clindamycin and/or erythromycin was found in isolates of serotypes Ia, Ib, II, III, V, VIII, and IX 4,17,18,31,33,45 . As a result of the increase in this resistance, a study reinforced the more recent guidance of the CDC (2010), which recommends carrying out susceptibility tests in the samples of GBS before treating colonized pregnant women 15 …”

“…3,14,15 Serotypes can be determined using techniques such as latex agglutination and polymerase chain reaction (PCR). 16,17 In the first technique, specific antibodies, available in reliable and easy-to-use commercial kits, are used for CPSs, whereas the PCR is based on the amplification of nucleotide sequence of the genes responsible for the capsular constituents. PCR has greater sensitivity and specificity in the identification of GBS than latex agglutination assay.…”

“…The identification of E. coli through PCR in placental tissues of pregnant women with risk factors for neonatal sepsis enhances the inclusion of the test in the Brazilian National Health System (SUS), mainly for women included in the risk group of sepsis. The test possesses accuracy and speed (ANDRADE et al, 2017) compared to the tests accomplished at the hospitals in patients with clinical signs of sepsis (DELLINGER et al, 2013). Besides, it can avoid the empiric use of antibiotics, because reflects the most probable pathogens, contributing with the knowledge of the local epidemiology and the causes of the sepsis (ALVES, 2011) being possible to optimize the prophylactic treatment in newborn, reducing the death risk and expenses.…”
