Modulation of oxidative stress and microinflammatory status by colloids in refractory dialytic hypotension

Abstract: BackgroundIntradialytic hypotension may adversely affect the outcome of chronic hemodialysis. Therapeutic albumin has powerful anti-oxidant and anti-inflammatory properties. We have recently shown that systematic colloid infusion during hemodialysis sessions improves hemodynamic parameters in most dialysis hypotension-prone patients unresponsive to usual of preventive measures. We postulated that frequent hypotensive episodes may lead to a noxious inflammatory response mediated by oxidative stress induced by is… Show more

“…Among the hypotheses that have been proposed to explain the positive effects of albumin, its antioxidant properties seem to be of paramount importance [11-14]. This hypothesis is supported by the central role of oxidative stress in critical pathologies, such as sepsis or liver failure [1,15].…”

“…The detrimental role of oxidative damage in critical pathologies, added to the demonstration of potential positive effects of albumin infusion in these patients, constitute a strong rationale that justifies for addressing the specific antioxidant capacities of HSA [11-14]. Our review summarizes the state of the art on this topic and focuses on the antioxidant properties of HSA related to (i) first its native form, (ii) the chemically modified-HSA (iii) the commercial HSA.…”

“…Although the practice of albumin infusion in severely depleted dialysis patients is probably more frequent, there are very few studies that address this question. [136,137]. As in many cases of severely ill patients, both beneficial and negative effects are difficult to ascertain.…”

“…While there are even fewer evidence-based studies on albumin infusion in dialysis patients, the few available ones suggest, as is logical to expect, that albumin infusion may have a favorable effect on intradialytic hypotension and a low risk of side effects. However, the advantages in the management of hypotension compared to saline infusion are not clear and probably cannot be generalized [135][136][137][138][139][140]. The high costs, the potential side effects, and, in particular, the risk of allergic reactions, need to be underlined.…”

“…Frequent episodes of hypotension may induce a noxious inflammatory response mediated by the oxidative stress [90,91]. It is supposed that abrupt fall in blood pressure accompanies systemic pathological condition which mimic acute ischemia reperfusion, inducing inflammatory type M1 macrophage activation [92].…”
