Objectives/Hypothesis Each year, the United States spends over $4 billion on emergency department visits for evaluation of dizziness. Benign paroxysmal positional vertigo (BPPV) is a common cause of dizziness that can easily be diagnosed by observing characteristic eye movements during the Dix‐Hallpike test (DHT). The DHT is easily performed; however, interpretation requires more advanced training. This may be part of the reason it is not commonly performed in emergency departments, and instead, patients undergo costly imaging tests. We evaluated whether smartphone‐based video recordings of DHT could be assessed telemedically for screening of nonacute dizziness. Study Design Feasibility study. Methods Dizzy patients underwent objective vestibular testing, but also had videos of their eye movements recorded via a smartphone during the DHT. The videos were remotely reviewed by two neuro‐otologists for BPPV screening and were compared to objective and in‐person exam findings. Results Overall, 30 dizzy patients were evaluated with seven cases of BPPV. The sensitivity for diagnosing BPPV via a smartphone‐recording of eye movements of the DHT was 92.86%, with a specificity of 100% and a negative predictive value of 97.87%. Conclusions Our initial proof‐of‐concept study shows that remote screening of BPPV is possible with high specificity. Because the DHT is easily taught, having a remote otolaryngologist interpret the resulting eye movements may increase usage of the test and may lead to cost savings. ZusAmmenfAssunG Telemedizin ist ein Oberbegriff, der alle kommunikationstechnologisch unterstützte Verfahren zusammenfasst, die medizinische Leistungen über räumliche Entfernungen erbringen. Dieser rasch sich entwickelnde Bereich der Medizin besitzt gerade für das stark endoskopisch/mikroskopisch ausgerich-tete Fach der Hals-Nasen-Ohrenheilkunde interessante Optionen. Für die klinische Untersuchung von Ohr, Nase, Rachen und Larynx, aber auch für die postoperative Nachsorge und die Betreuung nach Kopf-Hals-Tumor-Erkrankungen liegen Daten zum telemedizinischen Vorgehen vor, ebenso für die audiologischen Messungen nach Cochlea-Implantation. Diese Daten bieten z. T. ein interessantes Potential, z. T. weisen sie aber gerade auch auf Probleme bei unkritischer Anwendung der Telemedizin hin, deren Erfolg entscheidend von der Ausbildung des Untersuchers vor Ort abhängt. Der vorgestellte Artikel fasst die wesentlichen Erkenntnisse dieses sich sehr rasch entwickelnden Forschungsgebietes überblickartig zusammen. ABsTR AC TTelemedicine is a generic term summarizing all communication techniques assisting to provide medical services over distance. This rapidly developing medical area provides especially for the field of otorhinolaryngology with its high dependency on endoscopy and microcopy vaulable options. For clinical examination of ear, nose, throat and larynx, but also for postoperative care and surveillance of head and neck tumor patients data exist using the telemedical approach. Furthermore, audiological telemedical measurements even after Cochlear implantation are deemed feasible. While first data indicate interesting approaches, they also demonstrate problems, if a telemedical approach is used without proper preparation. Especially the observers at the remote location of the patients and their training are highly important in this regard. Telemedicine for head and neck ambulatory visits during COVID‐19: Evaluating usability and patient satisfaction

Background: In light of the COVID-19 pandemic, there has been a rapid increase in telemedicine visits. Otolaryngology patient satisfaction with these visits has not yet been extensively studied using a validated survey.Methods: All patients who had telemedicine visits with three head and neck surgeons, by phone or video-based platform, between March 25, 2020 and April 24, 2020. Retrospective chart reviews were conducted to determine demographic, disease, and treatment information. Patients who had a video visit were contacted by telephone and, if they could be reached and consented, were administered the telehealth usability questionnaire (TUQ). Results: Hundred surveys were completed. The average score across all questions was 6.01 on a scale from 1 to 7, where 7 indicated the highest level of patient agreement. The highest scores were for questions related to satisfaction with telehealth (6.29), while the lowest were related to reliability (4.86).Conclusions: Patients are generally highly satisfied with telemedicine.
