Mixed Capillary Venous Retroperitoneal Hemangioma

Abstract: We report a case of mixed capillary venous hemangioma of the retroperitoneum in a 61-year-old man. Abdominal ultrasonography showed a mass to be hypoechoic with increased flow in color Doppler imaging. Dynamic contrast-enhanced computed tomography revealed a centripetal filling-in of the mass, located anterior to the left psoas muscle at the level of sacroiliac joint. On the basis of imaging features, preoperative diagnosis of hemangioma was considered and the mass was excised by laparoscopic method. Immunohis… Show more

“…A few cases of retroperitoneal cavernous hemangioma were described as a cystic masses with minor or poor enhancement on enhanced CT (8). However, a case of retroperitoneal hemangioma with a capillary component exhibited a similar enhancement pattern to that of hepatic hemangioma (9). The present case also exhibited this enhancement pattern, which is expected because AH is a capillary subtype of hemangioma.…”

“…Among these, retroperitoneal hemangioma is extremely rare in adulthood, and has been confirmed in only 1% to 3% of all retroperitoneal tumors (8). The most common type of previously reported retroperitoneal hemangioma is the cavernous type (9). A few cases of retroperitoneal cavernous hemangioma were described as a cystic masses with minor or poor enhancement on enhanced CT (8).…”

“…Retroperitoneal tumours are best evaluated w i t h c r o s s s e c t i o n a l i m a g i n g a n d p r e -o p e r a t i v e histology (Strauss et al, 2011), but preoperative diagnosis of a retroperitoneal hemangioma based on imaging is extremely rare. Hemangioma essentially have no risk of malignant transformation (Godar et al, 2013). However, large hemangiomas have potential risk of rupture and massive haemorrhage (Peng et al, 2018) (Forbes, 2005), such as in our case.…”

“…Ultrasonography(US) imaging is usually the rst imaging investigation in a case of retroperitoneal tumour. US appearance and echogenicity of hemangioma can be variable, from hyperechoic to anechoic, typically with posterior acoustic enhancement (Godar et al, 2013).Contrast enhanced computed tomography(CECT) is the imaging of choice for a retroperitoneal tumour(Strauss et al, 2011)(Wee-Stekly & Mueller, 2014).CECT provides the tumour size, position, extent and adjacent organ involvement. Magnetic resonance imaging (MRI)is playing an increasing role in the evaluation of retroperitoneal soft-tissue masses due to its excellent tissue characterization and spatial resolution.…”

“…Primary retroperitoneal tumors are rare, accounting for only 0.07% to 0.6% of all tumors at the same location, and 62% to 82% of these tumors have been reported to be malignant [1]. Benign retroperitoneal tumors are rare, and vascular malformations such as hemangioma are exceptionally rare comprising only 0.5% to 2% of all retroperitoneal tumors [2].…”
