Minute pancreatic carcinoma with initial symptom of acute pancreatitis

Abstract: We experienced a case of minute pancreatic carcinoma in a 59-year-old man who complained of upper abdominal pain after drinking alcohol. Abdominal ultrasonography (US) revealed dilatation of the main pancreatic duct (MPD). Abdominal computed tomography (CT) and magnetic resonance cholangiopancreatography (MRCP) showed slight dilatation of the MPD and its obstruction near the portal vein. Endoscopic retrograde cholangiopancreatography (ERCP) demonstrated occlusion of the MPD, and cytology of aspirated pancreati… Show more

“…The cause of this tumorassociated pancreatitis is not known, but from a number of reports it is obvious that acute pancreatitis in association with a pancreatic neoplasm is not rare and occurs in 5% of pancreatitis patients. [30][31][32][33][34] It is assumed that duct obstruction by the tumor is an initiating factor in this type of pancreatitis.…”

“…Pancreatitis caused by pancreatic cancer occurs on occasion, with a reported rate of 3% (6). In general, acute pancreatitis is sometimes induced when the pancreatic duct is stenosed due to pancreatic cancer.…”

“…There are numerous causes of acute pancreatitis (22-44) ( Table 1 ) including alcohol ingestion, biliary tract disease (gallstones/malignancy), surgery/postoperative states (45), trauma (36), metabolic disorders (hypertriglyceridemia, hypercalcemia)(46-49), infections (adenovirus, mumps, ascariasis, mycoplasma) (44, 50), and drugs (the most common include thiazides, sulfa, methyldopa, and procainamide) (51, 52). Malignancies of the pancreas and bile ducts may result in pancreatitis due to obstruction/stenosis of the exocrine pancreatic ducts (53-57). Autodigestion caused by inappropriate intrapancreatic (instead of intestinal) activation of endogenous proteolytic enzymes has been proposed.…”
