Mini-Gastric Bypass for Revision of Failed Primary Restrictive Procedures: A Valuable Option

Abstract: Open MGB through a previous mini-incision is a safe and effective operation for revision of failed gastric restrictive operations. The revision procedure was technically more difficult in patients with prior VBG and hazardous in patients with prior redo VBG.

“…A long lesser curvature conduit and long biliary limb explain the physiology of modified Billroth II reconstruction, which has a non-obstructive and balanced restrictive–malabsorptive effect [17, 18]. Excellent results of mini-gastric bypass as a revision surgery have been reported after restrictive procedures (adjustable gastric banding, sleeve gastrectomy) [14, 15]. …”

“…In this term, different studies have assessed various lengths of the biliopancreatic arm based on small intestine length or patients' body mass index (BMI). Although studies have presented valuable results, their presentations are controversial and researchers have not declared unanimous results [11][12][13][14].…”

“…Cruroplasty [11], [12] and conversion to RYGB or OAGB/MGB have been described in GERD after SG [10]. OAGB/MGB as a gastric bypass surgery offers a safe and effective revisional surgery for failed gastric restrictive procedures [13]. Safety and long-term effects of OAGB/MGB are proven [14] and OAGB/MGB gives the opportunity to convert the high-pressure system into a low-pressure system, improving GERD and additionally including the option on further weight loss, which is not always achieved by RYGB [9], [15].…”
