Кравченко) ГБОУ ВПО «Омский государствен-ный медицинский университет» Минздрава России, Омск, Россия Цель исследования -оценка эффективности и преимуществ современных методов прерывания беременности в поздние сроки. Материал и методы. Обследованы 195 женщин, поступивших для прерывания беременности в сроке до 22 нед по медицинским показаниям (врожденные пороки развития плода, несовместимые с жизнью). В 1-ю группу вошли 98 бере-менных, у которых был применен комбинированный метод: мифепристон в дозе 200 мг однократно внутрь; через 24 ч вводились палочки ламинарии в цервикальный канал; если элиминация плода не происходила, через 3-4 ч назначался мизопростол в дозе 200-400 мкг. Во 2-й группе (97 беременных) прерывание беременности проводилось по схеме ВОЗ: применялись мифепристон 200 мг перорально, мизопростол 800 мкг вагинально или 400 мкг внутрь через 36-48 ч и затем по 400 мкг вагинально. Всем пациенткам проведено полное клиническое обследование в соответствии с действу-ющими стандартами. Результаты. Эффективность применяемых методов составила 99% в основной группе и 96,9% -в группе сравнения. Преимуществом комбинированного метода с применением палочек ламинарии была меньшая про-должительность времени от начала применения метода до появления сократительной деятельности матки в 1-й группе, чем во 2-й. Продолжительность прерывания беременности до 3 ч у беременных 1-й группы была зафиксирована в 12,2% наблюдений, во 2-й группе -в 1%. Применение меньших доз мифепристона и мизопростола привело к снижению неже-лательных (побочных) явлений (в 4,2 раза) в 1-й группе по сравнению с таковыми во 2-й группе. В 1-й группе в 2 раза меньше, чем во 2-й, был выражен болевой синдром. Определена прямая зависимость между наличием осложнений и длительностью процедуры медикаментозного прерывания беременности. Заключение. Применение комбинированного метода прерывания беременности в сроке до 22 нед по медицинским показаниям с использованием палочек ламинарии интрацервикально является более предпочтительным.Авторы информируют об отсутствии конфликта интересов.
