PurposeInjuries of the distal syndesmosis in ankle fractures are traditionally treated with a temporary adjusting screw fixation. Conventional fluoroscopic and X-ray examinations cannot reliably diagnose malpositions of the fixed tibiofibular syndesmosis. Postoperative computer tomography allows a reliable control of the transfixed region. The aim of the study was to clarify whether an intraoperative 3D image intensifier examination can detect malpositions of the syndesmosis already intraoperatively and whether the examination has an influence on the postoperative revision rate.MethodsIn 200 patients with tibiofibular syndesmosis injuries, an intraoperative 3D scan was performed after reduction of the distal tibiofibular syndesmosis and placement of the adjusting screw. Postoperative computer tomography of both ankle joints was performed in all patients.Results15% of all intraoperative 3D scans (30 patients) showed a finding requiring correction in the area of the ankle fork. In 7% of the cases, a malposition of the fibula in the tibial incisura requiring correction was found. Further corrections were necessary due to the extent and position of the osteosynthesis material (7%) and for the removal of joint bodies (1%). Postoperative computer tomographies of the ankle joints showed no deformities requiring revision.ConclusionAn intraoperative 3D scan allows a reliable assessment of the injured ankle region and reduces the postoperative revision rate. This makes a postoperative routine CT examination of the ankle joint dispensable.

FS Introducción El tratamiento de las fracturas de tobillo continúa siendo un foco de controversia cuyo origen radica en la variabilidad entre observadores a la hora de decidir qué fracturas pueden tratarse de forma ortopédica y cuáles necesariamente requieren cirugía, es decir, qué fracturas son estables y cuáles no. En los últimos años, existe una tendencia en alza hacia la indicación quirúrgica, motivada por la necesidad de una recuperación rápida y con menor riesgo de desarrollo de artrosis postraumática, pero entre los partidarios de la cirugía tampoco hay consenso en cuanto a qué estrategias de fijación utilizar para la sindesmosis y el resto de los estabilizadores, puesto que con los métodos clásicos (reducción indirecta, pinza de reducción para compresión y tornillo transindesmal) se han comunicado altas tasas de malreducción, por lo que aparece una nueva corriente que considera la fractura de tobillo como una fractura articular y que, por tanto, precisa reducción anatómica y fijación interna estable de todos sus fragmentos para permitir una movilización precoz. ZusammenfassungWährend Weber-C-Frakturen definitionsgemäß mit einer Verletzung der Syndesmose einhergehen, zeigen einige Studien, dass Weber-B-Frakturen mit Beteiligung der Syndesmose, bei adäquater Therapie mittels fibulotibialer Stellschraube, gleich gute klinische Ergebnisse aufweisen können wie solche ohne Syndesmosenverletzung. Aktuelle eigene Erhebungen zum klinischen und radiologischen Outcome bei isolierten Frakturen des Außenknöchels vom Typ Weber B mit oder ohne behandelte Syndesmosenverletzung bestätigen dies. Das Schmerzniveau, die klinischen Ergebnisse und die Röntgenaufnahmen zeigten keinen signifikanten Unterschied zwischen den Gruppen. Weber-B-Frakturen mit Ruptur der Syndesmose zeigten nur eine signifikante, aber klinisch nicht relevante Einschränkung der Dorsalextension im Vergleich zu denen mit intakter Syndesmose (15 vs. 20˚, p = 0,028). Schlussfolgerung: Die Ruptur der fibulotibialen Syndesmose beeinflusst die klinischen und radiologischen Outcome-Parameter nach Weber-B-Sprunggelenksfrakturen bei adäquater Behandlung mit einer fibulotibialen Stellschraube nicht.
