In addition to the positive effect of weight loosing after bariatric surgery, the post-operative period is often associated with signifi cant changes in bone metabolism.Aim. The aim of this study was to examine changes in bone turnover markers and indicators of dual-energy X-ray absorptiometry (DXA) within 12 months after laparoscopic greater curvature plication (LGCP).Materials and methods. We studied 54 patients from class III obesity who underwent LGCP. The average preoperative weight of the patients was 125.5±19.1 kg, and BMI of 43.0±4.9 kg/m 2 . The gender distribution of the cohort was 35.2% man and 64.8% women. At three time points after the operation (at 3, 6 and 12 months) control examination was conducted that included DXA and markers of bone metabolism.Results. After the surgical treatment we registered signifi cant reduction of all the anthropometric indicators (ρ<0.05; EWL 12 45.8±18.8%; EBL 12 55.1±23.4%). According to the controlled studies on the 12th months, a signifi cant increase in bone turnover markers was revealed (ρ<0.05; CTX, P1NP, OC, OPG, VTD) and reducing of PTH (ρ<0.05). The measure of DXA on the 12th month of observation showed no signifi cant changes in bone metabolism. Strong positive correlation was uncovered between markers of bone metabolism: CTX / P1NP (r=0.790, p=0.000), CTX/ OC, P1NP/OC (r=0.7, p=0.000), minor correlation between PTH/OC (r=0.249, p=0.027). We also report a minor and weak negative correlation between the markers of bone metabolism (CTX, P1NP, OC and VTD) and anthropometric data.Conclusions. Twelve months post LGCP markers of bone turnover (CTX, P1NP, OC, OPG, VTD) were signifi cantly increased. The increased level of VTD is associated with the loss of FAT. Expression of vitamin D-metabolizing enzymes in human adipose tissue—the effect of obesity and diet-induced weight loss

OBJECTIVE: Low vitamin D (VD) levels are common in obesity. We hypothesized that this may be due to metabolism of VD in adipose tissue (AT). Thus, we studied (1) whether the VD-metabolizing enzymes were expressed differently in AT of lean and obese individuals and in visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT), and (2) whether their expression was influenced by weight loss. METHODS: Samples of SAT and VAT were analyzed for expression of the vitamin-D-25-hydroxylases CYP2R1, CYP2J2, CYP27A1 and CYP3A4, the 25-vitamin-D-1a-hydroxylase CYP27B1, the catabolic vitamin-D-24-hydroxylase CYP24A1, and the vitamin D receptor, using reverse transcriptase-PCR. Moreover, plasma 25-hydroxy-vitamin D (25OHD) level was measured and related to the expression of these enzymes. Samples of SAT and VAT from 20 lean women and 20 obese women, and samples of SAT from 17 obese subjects before and after a 10% weight loss were analyzed. RESULTS: A plasma 25OHD level o50 nmol l À 1 was highly prevalent in both lean (45%) and obese (90%) women (Po0.01). Plasma 25OHD increased by 27% after weight loss in the obese individuals (Po0.05). Expression levels of the 25-hydroxylase CYP2J2 and the 1a-hydroxylase CYP27B1 were decreased by 71% (Po0.0001) and 49% (Po0.05), respectively, in SAT of the obese. CYP24A1 did not differ between lean and obese women, but the expression was increased by 79% (Po0.05) after weight loss. CONCLUSION: Obesity is characterized by a decreased expression of the 25-hydroxylase CYP2J2 and the 1a-hydroxylase CYP27B1 in SAT, whereas the catabolic CYP24A1 does not differ between lean and obese women. However, the expression of CYP24A1 is increased after weight loss. Accordingly, AT has the capacity to metabolize VD locally, and this can be dynamically altered during obesity and weight loss. Abstract: Postoperative values of vitamin D show a progressive increase, while PTH ones present a significant reduction, without any impact on serum calcium levels. We have demonstrated an inverse correlation between weight loss and vitamin D increase at the third month after surgery.
