Microfracture Versus Autologous Chondrocyte Implantation for Articular Cartilage Lesions in the Knee: A Systematic Review of 5-Year Outcomes

Abstract: Patients undergoing MFx or first/third-generation ACI for articular cartilage lesions in the knee can be expected to experience improvement in clinical outcomes at midterm to long-term follow-up without any significant difference between the groups.

“…Bone marrow stimulation using the microfracture (MF) technique and autologous chondrocyte implantation (ACI) are currently used as joint-resurfacing procedures. 1,2 MF is a singlestage arthroscopic procedure that has gained popularity during the past 2 decades because of its minimally invasive approach, technical simplicity, low surgical morbidity, and relatively low cost. 2 Although excellent short-term clinical outcomes (first 24 months) have been demonstrated after bone marrow stimulation, the clinical durability of bone marrowestimulated repair tissue has shown an objective and functional decline over time.…”

“…Articular cartilage injury and degeneration brings serious economic burden to their families and the society (30). The rise of ACI technology has brought hope for rehabilitation of these patients and is currently considered to be the best treatment for cartilage injury (31). Although stem cells such as bone marrow mesenchymal stem cells, embryonic stem cells, pluripotent stem cells, and umbilical cord blood stem cells can be used as seed cells, the use of stem cells has many problems, such as ethical factors, differentiation instability, and cancer risk (32).…”
