Superselective embolization is effective in controlling colonic hemorrhage and is associated with a low rate of postembolization ischemia. Our experience with angiography reinforces the paradigm shift from its use as a diagnostic tool to the primary method for the control of colonic bleeding. Because patients have been followed up for as long as seven years, this approach remains effective for the long-term treatment of colonic bleeding.

Four cases of extraperitoneal haemorrhage occurring in the setting of anticoagulation or coagulopathy are presented. Treatment of this condition has traditionally consisted of reversal of anticoagulation and supportive therapy. Diagnosis was made on contrast-enhanced computed tomography. The finding of active contrast extravasation was found to be a factor predictive of failure of conservative therapy and, therefore, an indication for angiography and embolization. In all four cases presented in the present paper, the bleeding vessel(s) were identified and significant active bleeding arrested by transcatheter embolization.

Therapeutic embolization for acute haemorrhage is increasingly being utilized. An audit of 34 patients undergoing therapeutic embolization for acute abdominal or pelvic haemorrhage was undertaken, in an attempt to assess the importance of the following variables in determining a successful outcome: coagulation status, transfusion status, time to procedure after onset of circulatory instability, duration of procedure, and the effect of the embolization technique employed. Overall success was 79%, with definitive control of haemorrhage achieved by embolization; 21% required surgical management for rebleeding 4-24 h post embolization. The duration of the procedure and transfusion status of the patient were the most important factors associated with a successful outcome.
