A 34-year-old Sri Lankan man presented with multiple episodes of vomiting following accidental ingestion of 100 ml of indoxacarb (Avaunt). He had a significant saturation gap (discrepancy between oxygen saturation [98%] in blood gas analysis and saturation on pulse oximetry [70%] in finger pulse oximetry), the color of the blood was muddy brown, and the methemoglobin level (60%) was high in the blood gas analysis. A diagnosis of methemoglobinemia secondary to indoxacarb poisoning was made, and he was treated with methylene blue with a favorable outcome. Methemoglobinemia secondary to indoxacarb poisoning is extremely rare, and clinicians should be aware of this important complication. Methemoglobinemia secondary to indoxacarb poisoning has a favorable outcome if recognized and treated promptly.

Indoxacarb is a oxadiazine group of insecticide and its toxicity in human is not fully reported. We are reporting a case of 30 years old urban female who presented with a triad of altered consciousness, deep cyanosis and low SpO 2 and markedly high PaO 2 after ingestion of unknown insecticide with suicidal intent. On strong clinical suspicion of indoxocarb poisoning, we treated her for methaemoglobinaemia with methylene blue and patient recovered fully.

A 17 year old male developed adult respiratory distress syndrome (ARDS), seizures, intravascular hemolysis and peripheral neuropathy following self-poisoning with a pesticide containing 14.5% indoxacarb. He was managed conservatively with fluid management, antibiotics, mechanical ventilation and physiotherapy. There is limited data concerning the human toxicity of indoxacarb. This case demonstrates that findings from animal studies, such as seizures, respiratory distress, and intravascular hemolysis are possible following self-poisoning in humans. In this patient, partial recovery was achieved following supportive care. Key Words: ARDS, indoxacarbs, neurotoxicity, orange urineÖzet 17 yaşındaki bir erkekte % 14.5 indoksakarb içeren bir pestisit ile zehirlenmeyi takiben ARDS, nöbet, intravasküler hemoliz ve periferik nöropati gelişti. Sıvı tedavisi, antibiyotikler, mekanik ventilasyon ve fizyoterapi ile konservatif olarak tedavi edildi. İndoksakarbın insanlardaki toksisitesi ile ilgili sınırlı veri bulunmaktadır. Bu vaka hayvan çalışmalarından nöbet, solunum sıkıntısı ve intravasküler hemoliz gibi bulguların zehirlenmeyi takiben insanlarda olabileceğini göste-rir. Bu hastada destekleyici bakım sonrasında kısmi iyileşme sağlan-mıştır.
