Metastasis of femoral osteosarcoma to the abdominal wall detected on 99m Tc-MDP skeletal scintigraphy

Abstract: Osteosarcoma is the most frequent primary malignancy of bone, and usually metastasizes to the lung and bones, while other sites are rare. In most reported cases, soft tissue metastasis of osteosarcoma is unusual, and only develops in the advanced stages of the disease, especially following multiple recurrences. We present a patient with recently diagnosed osteosarcoma of the right femur, showing abdominal wall metastasis diagnosed by technetium-99m-methylene diphosphonate (99m Tc-MDP) whole body bone scintigra… Show more

“…However, the CT counterpart of the image did not demonstrate any calcification, which suggests cellular-level microscopic calcifications. Soft tissue calcific metastases concentrating bisphosphonates have been reported previously [4], and similar reports are also evident with 18 F-NaF [5][6][7]. Nonosseous uptake of 18 F-NaF due to heterotrophic ossification, brain metastases, metastatic lymph nodes [6] and liver metastasis [7], as well as atherosclerosis [8], has been reported.…”

“…Abdominal metastases represent a poor prognostic indicator, with a median survival after diagnosis of approximately 2-3 years [3]. Osteosarcoma is an aggressive tumor [3] and is characterized by a metastatic pattern dominated by pulmonary metastases (98%) followed by bone metastases (37%) [1,3], abdominal metastases are very rare [4]…”

“…The survival rate of osteosarcoma is closely related to tumour metastasis in that 15-20% of patients were diagnosed with distant metastatic lesions at their first visit [14]. Previous studies have recognized that the lung is the most sensitive site, but the occurrence of abdominal metastasis reveals an unusual pattern in which osteosarcoma is more prone to occur in soft tissue [15]. Changes in the tumour microenvironment and extracellular matrix composition seriously affect osteosarcoma metastasis, e.g., the "seed and soil hypothesis".…”
