Metabolic Syndrome and Psoriasis: Mechanisms and Future Directions

Abstract: Psoriasis is an immune-mediated systemic disease with associated comorbidities, including metabolic syndrome (MetS) which contributes substantially to premature mortality in patients with psoriasis. However, the pathological mechanisms underlying this comorbidity are unclear. Studies have shown that the pathological parameters of psoriasis mediate the development of MetS. We reviewed the potential mechanisms which mediate the association between psoriasis and MetS, including endoplasmic reticulum stress, pro-i… Show more

“…Inflammation in psoriasis is perpetuated by many factors, namely dysregulated native and adaptive immune response, increased oxidative stress, altered balance of adipokines, insulin resistance, and endothelial dysfunction [ 17 , 20 , 21 ]. Insulin resistance is commonly present in patients with psoriasis and contributes to the progression of psoriatic disease.…”

“…Furthermore, inflammation-induced insulin resistance increases endothelial expression of adhesion molecules, which promotes further inflammation in psoriatic plaques [ 23 ]. Proinflammatory cytokines, whose production is markedly increased in psoriasis (TNF-α, IL-17, IL-23, and IL-6) all contribute significantly to insulin resistance in patients with psoriasis [ 21 , 24 ].…”

“…This may be compounded by the increased risk of metabolic syndrome in psoriasis with considerable immunological overlap and systemic inflammation seen in both disorders [39]. Metabolic syndrome is associated with increased risk of major adverse cardiac events and the risk of metabolic syndrome is higher in patients who are heavy drinkers > 35 g/day (RR 1.86) [39,40]. Psoriasis patients are also at increased risk of AF with an HR of 1.42 compared with controls [41].…”

“… 1 , 2 Although certain progress has been made in the treatment of psoriasis, this disease still cannot be completely cured and often leads to many complications, such as psoriatic arthritis, cardiovascular disease, metabolic syndrome, uveitis, etc. 3 , 4 , 5 , 6 In addition to these complications, the body appearance of the psoriasis patient is also affected 7 , 8 ; all of these factors contribute to a huge psychological burden (such as anxiety and depression) on the psoriasis patients, which causes difficulties in rehabilitation and even leads to suicide. 9 , 10 , 11 Therefore, it is necessary to explore potential factors that participate in the pathology of anxiety and depression, which might be helpful for attribution therapy and thereby improve the clinical outcomes of psoriasis patients.…”
