Background: Neurodevelopmental disorders (NDD) and mental health (MH)-related conditions among children and adolescents, represent a global public health burden, that was worsened by the COVID-19 pandemic due to disruptions in access to healthcare, support services, and increased stressors exacerbating symptoms and challenges. However, few studies have examined in France, NDD and MH’s diagnoses and care pathway in youth attending public healthcare center (HCC) services. Methods: An annual cross-sectional analysis was performed in youth aged 0 to 17 years on health information, that were extracted from the French PMSI database (DIAMANT database) between January 1, 2014, and December 31, 2022, using the MH-related ICD-10 diagnosis codes, including NDD, mood-, behavior-, anxiety-, and other-disorders (sleep and eating). Trends in healthcare center visits for NDD and MH-related conditions, were evaluated according to age, sex, diagnoses, and care pathway over the 10-year period using ANOVAs. Findings: Out of the 17,707,806 total HCC-visits across a 10-year period, 289,382 visits (1,6%) were related to youth with diagnoses for NDD or MH disorders of the study interest. Among them, 57% were girls and 43% boys. The total number of pediatric HCC-visits related to NDD and MH has remained stable from 2014 to 2022. However, the proportion of visits has doubled for adolescent girls with internalizing disorders (anxiety and mood) while a 5-fold decrease in the proportion of visits for NDD, was observed for boys aged 0 to 12 years. The HCC-admission mainstream for internalizing disorders was the emergency department and was “home” for the NDD. While the HCC-discharge accounted for 9% of patients being hospitalized, most of them were sent home (90.7%) with a strong age- and sex-specificities. Interpretation: Over the last 10 years, the proportion of HCC visits for NDD and MH remained stable, but strong age- and sex-effects were found on diagnoses visits and care pathway. These findings are essential to improve emergency and outpatient service capacity dedicated to youth. They could help European and national policies to patient-tailor care pathway depending on sex and diagnosis from screening, interventions, and monitoring during this sensitive period of development.

Families can spend years waiting for a developmental assessment. We sought to understand supports caregivers had accessed by the time of their child's first multi‐disciplinary developmental assessment, the supports caregivers wanted, and the barriers caregivers reported to accessing these supports. We also sought to understand how government funding schemes (the National Disability Insurance Scheme) and sociodemographic factors influenced access to supports. Caregivers were emailed questionnaires on sociodemographic background and intervention history prior to their child's developmental assessment at a tertiary diagnostic and assessment service. Results from 202 caregivers showed most children were receiving less than 2 hours of support each week at assessment. The most common accessed supports were from general practitioners and pediatricians. Caregivers reported behavioral therapists (41%) and psychologists (29%) as the most desired but inaccessible child supports. Half of caregivers nominated a need for parenting interventions. The most frequent barriers to accessing child supports were wait lists, finance, and knowledge. Government funding improved access to the total number of services received (from 2.7 to 5.2 different services), but for disability specific services only (e.g., speech and occupational therapy). Results highlighted disparities for families without government funding, impacting certain groups (e.g., children over 7 years) and services (e.g., mental health). Socioeconomic disadvantage, parental separation, and, surprisingly, being from a non‐culturally/linguistically diverse background were associated with fewer services and more barriers. Results highlight the need to facilitate access to supports for families to align with existing recommendations.

Impact of Child and Family Factors on Caregivers’ Mental Health and Psychological Distress during the COVID-19 Pandemic in Greece

Although primary caregivers of children with developmental disabilities (DDs) experience higher levels of distress than primary caregivers of typically developing children do, this problem has received limited attention in Greece. Therefore, this study examined mental health and associated factors among primary caregivers of children with and without DDs in Greece during the COVID-19 pandemic. This cross-sectional study included 156 participants. Primary caregivers completed a self-report survey on sociodemographic characteristics, the Depression, Anxiety, and Stress Scale-21 items, and the 12-item General Health Questionnaire. Primary caregivers of children with DDs, particularly mothers, reported more mental health problems and higher levels of psychological distress than the control group. Among families parenting a child with disabilities, caregivers’ psychological distress was significantly related to having a child with autism spectrum disorder and the severity of the child’s behavioral difficulties. Significant predictors of caregivers’ distress were the parent being female, the child being male, a single-parent family, a lower income, and higher depressive symptoms. Caregivers raising children with DDs face unique challenges in terms of care, necessitating the development of family-based interventions to improve the social-emotional well-being and overall quality of life for both parents and children.
