Clinical assessment of skin photosensitivity is subjectively determined by erythema and tanning responses to sunlight recalled by the subject, alternatively known as Fitzpatrick Skin Phototype (SPT). Responses may be unreliable due to recall bias, subjective bias by clinicians and subjects, and lack of cultural sensitivity of the questions. Analysis of red-green-blue (RGB) color spacing of digital images may provide an objective determination of SPT. This paper presents the studies to assess the melanin index (MI), as determined by RGB images obtained by both standard digital camera as well as by videodermoscope, and to correlate the MI with SPT based upon subjects' verbal responses to standardized questions administered by a dermatologist.A sample of subjects representing all SPTs I-VI was selected. Both the digital camera and videodermoscope were calibrated at standard illumination, light source and white balance. Images of constitutive skin of the upper ventral arm were taken of each subject using both instruments. The studies showed that 58 subjects (20 M, 38 F) were enrolled in the study (mean age: 47 years; range: 20-89), stratified to skin phototype I-VI. MI obtained by using both digital camera and videodermoscope increased significantly as the SPT increased (p = 0.004 and p < 0.0001, respectively) and positively correlated with dermatologist-assessed SPT (Spearman correlation, r = 0.48 and r = 0.84, respectively). Digital imaging can quantify melanin content in order to quantitatively approximate skin pigmentation in all skin phototypes including Type VI skin. This methodology holds promise as a simple, non-invasive, rapid and objective approach to reliably determine skin phototype and, with further investigation, may prove to be both practical and useful in the prediction of skin cancer risk.

Sažetak:Zdravstvena pismenost predstavlja stepen u kojem pojedinci imaju sposobnost da prime, obrade i razumeju osnovnu zdravstvenu informaciju i usluge neophodne da se donesu odgovarajuće zdravstvene odluke. Rak kože se u velikoj meri može sprečiti izbegavanjem izlaganja nebezbednom ultraljubičastom zračenju. Zdravstvena pismenost poprima sve veću pažnju, ali njen uticaj na donošenje odluka o sunčanju još uvek nije dovoljno istražen. Cilj rada: Istražiti i prikazati metode za procenu zdravstvene pismenosti pacijenata u vezi sa rakom kože i zaštite od sunca, a specifičan cilj je analiza instrumenata za kvantitativnu procenu zdravstvene pismenosti. Materijal i metode: Prikupljanje publikacija u vezi sa zdravstvenom pismenošću pacijenata i rakom kože izvršeno je pretragom literature, a za svaki instrument kojim se vrši procena zdravstvene pismenosti ispitanika u vezi sa rakom kože dat je njegov sadržaj i opis. Metodom komparacije urađeno je poređenje tri instrumenta od ukupno deset koji su opisani u našem radu. S-TOFHLA, CHLCC i Braun i Clarke (2006) metode 6-fazne tematske analize su instrumenti korišćeni za analizu zdravstvene pismenosti ispitanika u vezi sa rakom kože. Međutim, u dva rada su iskorišćena tri pitanja preuzeta iz S-TOFHLA testa sa ciljem procene opšte zdravstvene pismenosti pacijenata. Najčešće je korišćena i kvantitativna i kvalitativna analiza. Naš rad pokazuje da postoji inicijativa u svetu za podizanjem svesti i zdravstvene pismenosti u vezi sa rizikom od oboljevanja od raka kože preko korišćenja nekoliko tipova instrumenata, što rezultuje promenom stavova u vezi sunčanja i sticanja tena i namerom o promeni ponašanja u vezi zaštite od izlaganju suncu. Ključne reči: zdravstvena pismenost, rak kože, metode za procenu zdravstvene pismenosti pacijenata u vezi sa rakom kože i zaštite od sunca. Summary:Health literacy is the degree to which individuals have the capacity to obtain, process, and understand basic health information and services needed to make appropriate health decisions. Skin cancer can largely be prevented by avoiding exposure to unsafe ultraviolet radiation. The attention given to health literacy is growing, but its impact on decision-making on sunbathing has not yet been sufficiently researched. Aim of the study: To explore and show the methods for the assessment of health literacy of patients in relation to skin cancer and sun protection, and the specific aim is the analysis of instruments for the quantitative assessment of health literacy. Materials and Methods: The collection of publications related to health literacy of patients and skin cancer was performed by literature search, and the content and description were given for each instrument that used for the estimation of health literacy of respondents regarding skin cancer. S kin pigmentation and sun sensitivity vary widely among US Hispanics, 1 whose median number of nevi (the strongest melanoma risk factor) is somewhat lower than in whites yet higher than in Asians or blacks. 2 The correlation between number of nevi and age is stronger in Hispanics and non-Hispanic whites than in other ethnoracial groups. 2 Among Hispanics, acculturation to the United States might lead to decreased sun safety practices. 3 Nationwide data from 1992 through 2007 reveal that melanoma incidence among Hispanics increased by more than 22%. 4,5 Hispanics display higher rates of thick melanoma at diagnosis, and in the absence of cure, targeted prevention might be the best strategy for countering the epidemic. 6 Hence, our objective was to synthesize the evidence about skin cancer screening among US Hispanics. Methods. We identified observational populationbased US studies on melanoma or other skin cancer screening that evaluated participants of Hispanic descent, without any age, time, or language restrictions. Hispanic or Latino ethnicity was defined as Mexican, Puerto Rican, Cuban, or Central or South American heritage regardless of race. 7 Screening techniques included skin self-examination (SSE), clinical skin examination (CSE), dermoscopy, and biopsy. We conducted an extensive literature search through October 2010 using MEDLINE (from 1950), EMBASE (from 1974), CancerLit (from 1963), and Lilacs (from 1982) and reviewed the bibliographies of all relevant articles. The following keywords and indexing terms were used: melanoma, skin neoplasms, self-examination, early detection of cancer, and mass screening. From the 1029 retrieved articles, we excluded duplicates, reviews, non-US studies, and those with patient or survivor samples, selecting 138 articles for detailed review. Studies with missing ethnoracial data were excluded. Nine studies met all inclusion criteria, and from each we extracted the age range, population type, health care access status, setting, number and/or percentage of Hispanics with reported melanoma or other skin cancer screening, year of assessment and measurement method.
