The aim of the present study is to display the clinical and dermoscopic features of melanocytic nevi of more than 5 mm in diameter in psoriasis patients. A total of 32 patients with psoriasis (21 male, 11 female; average age 37.4) formed the first study group. In 22 patients (68.8%), melanocytic nevi of 5 mm in diameter and bigger (total of 68 nevi) were clinically found. For a randomized trial, 100 people (21 male, 79 female; average age 27.8) without psoriasis were invited to form the second study group. Only 37 of them had nevi ≥5 mm in diameter (total of 60 nevi). Complete questionnaire, full body photometric skin examination, dermoscopy examination on the dermatoscope HEINE MINI 10X with 70% ethyl alcohol immersion, skin type identification according to the Fitzpatrick classification, and nevi assessment according to ABCD and ABC rules were obtained for all recruited people. Our study showed that patients with psoriasis are more susceptible to melanocytic nevi of >5 mm in diameter (68.8%), while the result for the second group was 37%. As for nevi of dysplastic criteria, we found 32% in psoriasis patients vs. 42% in the monitoring group. Moreover, we should bear in mind the influencing factors of skin phototype II and artificial insolation (e.g., tanning, PUVA (Psoralen and ultraviolet A), and narrowband phototherapy 311 nm) obtained by the patients from the first group.

The review presents modern data on the role of ultraviolet (UV) radiation in the pathogenesis of non-melanoma skin cancer (NMSC), the problem of THE risk of developing NMSC, in particular, squamous cell and basal cell skin cancer both in the population and in long-term repeated irradiation of phototherapy (PUVA therapy, UVB therapy, UVB-311 therapy) in patients with psoriasis. The paper considers the mechanisms of UV-induced cell damage by different spectral ranges (UVA, UVB) including the formation of photoproducts, damage to genomic DNA and other cellular structures, violation of the regulation of signaling pathways, the development of chronic inflammation, secondary immunosuppression. The review summarizes the results of large epidemiological studies discussing the role of gene polymorphisms in the homologous DNA repair XRCC3, gene telomerase TERT-CLPTMI, cytokine IL10 gene, MTHFR gene, encoding the folate synthesis, genes involved in pigmentirovanie MC1R, EXOC2, UBAC2 in the modulation of risk of carcinogenic effect of UV radiation. According to the authors’ opinion, the most vital and significant is data on the role of vitamin D receptor (VDR) gene polymorphisms as possible predictors of the risk of NMSC development. The further prospects of academic research on the cumulative role of the genome and environmental factors in the risk assessment of NMSC are revealed.

Dermatolojik tedavide son yıllarda önemli gelişmeler olmaktadır. Dermatoloji tarihindeki en eski tedavi yöntemlerinden biri olan fototerapi, bu gelişmelerin gerisinde kalmayıp, tam tersine, etkin ve güvenilir olmasının avantajıyla, pek çok deri hastalığının tedavisinde yaygın olarak kullanılmaya devam etmektedir. Bu makale, mevcut fototerapi yöntemlerinin tarihçesinden başlayarak, yapay ultraviyole kaynakları, bu kaynakların dozimetre ve kalibrasyon standartları ve ultraviyole tedavi protokolleri konusunda pratik ve temel bilgiler verebilmeyi hedeflemektedir. Fototerapinin TarihçesiFototerapinin tarihçesi binlerce yıl öncesine dayanır. 3500 yıl önce Mısır'da, o zaman lepra olduğu sanılan vitiligoda, Nil deltasında yetişen ve psoralen içeren bir bitki olan Ammi majus'u kaynatarak içtikten sonra güneşe çıkmak bir tedavi yöntemiydi. Modern fototerapide ilk adımlar, "fototerapinin babası" olarak bilinen Niels Ryberg Finsen tarafından atılmıştır. Güneş ışığının antibakteriyel etkileri olduğunu farkeden Finsen, yapay bir ultraviyole (UV) lambası geliştirmiş ve bu "Finsen lambası" ile, 800'den fazla lupus vulgaris hastasını tedavi ederek, %80'inde kür sağlamış, bu buluşuyla 1903 yılında Nobel ödülü kazanmıştır. Dermatoloji tarihinde Nobel ödülü alan ilk ve tek kişidir (1-3).Fototerapi alanına yaptığı katkılarla ismi anılması gereken isimlerden bir diğeri de Goeckerman'dır. Goeckerman, Mayo klinikte, halen kendi adıyla anılmakta olan bir fototerapi yöntemi geliştirmiş ve elde ettiği ilk sonuçları 1925 yılında yayınlamıştır.
