Blepharospasm and oromandibular dystonia are focal dystonias characterized by involuntary and often patterned, repetitive muscle contractions. There is a long history of medical and surgical therapies, with the current first-line therapy, botulinum neurotoxin (BoNT), becoming standard of care in 1989. This comprehensive review utilized MEDLINE and PubMed and provides an overview of the history of these focal dystonias, BoNT, and the use of toxin to treat them. We present the levels of clinical evidence for each toxin for both, focal dystonias and offer guidance for muscle and site selection as well as dosing.

Background The features of non-motor symptoms such as cognitive ability, psychiatric state, and quality of life (QOL) is unknown in patients with jaw closing dystonia. Methods Seven patients with jaw closing dystonia (2 males, 5 females; mean age 60.4 years) and 7 healthy age-and gender-matched subjects (mean age 54.1 years) participated in this study. The patients were examined to determine the location of pain and movement difficulties, with awareness of those measured using a numerical rating scale (0-10).

that oromandibular dystonia (also referred to as orofacial dystonia) is a movement disorder causing paroxysmal or sustained contraction of the affected facial muscles. Oromandibular dystonia may occur alone or as a part of a segmental or generalized dystonia and may be idiopathic or secondary to previous medication or drug use, trauma, or neurodegenerative disease. The AAOM also affirms that patients with suspected oromandibular dystonia should be referred to the appropriate oral and/or medical health care provider(s) for comprehensive evaluation and management of the condition. Management of oromandibular dystonia may include conservative therapies, medications, botulinum toxin injections, or peripheral or central surgical approaches. This Clinical Practice Statement was developed as an educational tool on the basis of expert consensus of the American Academy of Oral Medicine (AAOM) leadership. Readers are encouraged to consider the recommendations in the context of their specific clinical situation and to consult, when appropriate, other sources of clinical, scientific, or regulatory information before making a treatment decision.
