Medication Safety

Abstract: Pharmacotherapy is the most common therapeutic intervention in healthcare to improve health outcomes of patients. However, there are many instances where prescribed medications resulted in patient morbidity and mortality instead. Medication errors can happen at any step of the medication use process, but a substantial burden of medication-related harm is focused primarily on three priority areas of healthcare delivery: transitions of care, polypharmacy and high-risk situations. This chapter highlights prevalen… Show more

“…Management of complex drug therapy is safer and more sustainable whenever it can be streamlined for application in the real-world environment. 5 Successful carboplatin desensitization must be followed by use of the desensitization schema with each subsequent chemotherapy cycle containing carboplatin contingent on demonstrated patient tolerance to this method of drug administration and the premise that immunologic resensitization may occur during the interval between cyclic carboplatin administrations. 2,4,9 Effective desensitization is accomplished by controlled and uninterrupted carboplatin administration that delivers an increasing intensity of drug exposure to ultimately administer the regimen-based dose.…”

“…[2][3][4] Interdisciplinary planning, order input, and implementation of every desensitization schema step must be done in a meticulous manner for delivery of safe and appropriate patient care. 2,5 Desensitization schemas are complex and add significantly to the workload of interdisciplinary staff. System adoption of a standard approach to chemotherapy desensitization is helpful for providing safe and seamless patient care, but widespread implementation is limited by the complexity of these protocols.…”

“…Therefore, nurses must understand how polypharmacy and pill burden occur and affect patients' health outcomes. In geriatric patients, the liver size decreases with age, and its ability to metabolize drugs also declines, which increases the risk of toxicity (7).…”

“…Simplifying the delivery of patient care improves medication safety and regimen adherence. 6 The purpose of this commentary is to evaluate an empirical rationale for dexamethasone administration according to a standard schedule with elotuzumab-containing myeloma regimens.…”
