Objectives Pharmacists constitute a crucial component of the healthcare system, significantly influencing the provision of medication services and ensuring patient safety. This study aims to understand the characteristics and risk factors for complaints against pharmacists through Health and Disability Commissioner (HDC) published decisions. Methods This study adopts a retrospective, qualitative approach. An inductive content analysis technique was used to analyze 37 complaints against pharmacists published decisions from the New Zealand Health and Disability Commissioner website to investigate a range of underlying risk factors contributing to the occurrence of complaints against pharmacists. Results A set of 20 categories of risk factors emerged through the content analysis and were subsequently grouped into five overarching themes: pharmacist individual factors, organizational factors, system factors, medication-specific factors, and external environmental factors. Conclusions The findings of this study provide valuable insights that expand the understanding of risk management in pharmacist practice, serving as a valuable resource for regulatory bodies, policymakers, educators, and practitioners. It is recommended not only to focus solely on individual pharmacists but also to consider integrating their environment and individual behaviors to proactively address situations prone to errors and subsequent complaints.

Medicine has been a noble profession since time immemorial. Many cultures have the view that it is linked with the supernatural deity. The persons were priests in many civilizations. They had high status in Society. They were considered as a medium of communication between the supernatural and were assumed to have powers. In those times, those priests used rituals and follow some religious code of conduct for treatment. This was the primitive stage of ethics as a part of religious ritual. Latter with observations, many folk traditional medicine ethics were created in a teacher-student relationship. Further traditional medicine like Ayurveda, TCM, Korean developed, which had mutual interactions in ideology and philosophy. It was evident with the review of these studies and also had a diverse set of rules entertaining behavioral and professional codes for prescriber health care worker and patient for providing rational and safe delivery of health services. However, with the separation of medical science from religion, the positive philosophical background and the behavioral element lost its ground. Thus rationality becomes behavioral In addition, previously, all elements of treatment were done by viadys or healers, but with development, different professions arrived, which incensed scope of error, which is a hurdle to the objective of rational drug use in of drug. Hence, a universal approach of PDAA CYCLE and DRIOP methodology concept to optimize teamwork and ensure rational drug use is conceptualized. This approach has taken into consideration of all elements in the drug use process and has been devised to bridge the gaps leading to irrational use in the modern scenario. This would be helpful in the rational use of widely used drugs like antibiotics, low therapeutic index drugs, chronic illness drugs and also help to improve antibiotic resistance and lead to optimum utilization of resources, which is vital for every country but especially for underdeveloped countries and developing countries like India with high load of infections and other diseases.

ABSTRAKKejadian Nyaris Cedera (KNC) yang disebabkan oleh dispensing error pada instalasi farmasi RS di tempat studi terjadi pada dua bulan berturut-turut pada Januari dan Februari 2015. Tujuan penelitian ini adalah untuk mengidentifikasi akar masalah dispensing error tersebut sehingga terjadi KNC, identifikasi alternatif-alternatif solusi, pemilihan solusi dan implementasi solusi yang terpilih. Metode penelitian yang dilakukan adalah penelitian tindakan menggunakan bauran metode yang melewati tahapan-tahapan diagnosa, recconnaissance, rencana tindakan, tindakan, dan evaluasi. Diagnosa masalah adalah KNC yang disebabkan oleh dispensing error. Tahap recconnaissance dilakukan dengan mengumpulkan data melalui wawancara, diskusi kelompok terarah, pengamatan, dan telaah dokumen. Wawancara tidak terstruktur melibatkan 6 orang terdiri dari manajer, dokter, dan staf. Diskusi kelompok terarah melibatkan pimpinan rumah sakit, jajaran manajemen, dan kepala instalasi farmasi. Observasi dilakukan terhadap fasilitas fisik dan layanan resep. Hasil analisis data dituangkan dalam bentuk bagan tulang ikan. Faktor akar penyebab masalah terjadinya dispensing error adalah belum terformulasikannya kebijakan yang kemudian berdampak pada klaster masalah individu, tim, fasilitas, proses layanan, dan komunikasi. Jalan keluar utama dari berbagai penyebab akar permasalahan tersebut adalah formulasi kebijakan. Formulasi kebijakan menciptakan perbaikan pada prosedur layanan farmasi rawat jalan yang meliputi penerapan prinsip penyimpanan obat LASA, dan alur layanan farmasi rawat jalan yang berfokus pada pencegahan dispensing error meliputi perbaikan alur layanan mulai dari prosedur peresepan, penerimaan resep, penyiapan, etiketing, komunikasi informasi dan edukasi (KIE) obat sampai dengan prosedur penyerahan obat. Prosedur tersebut dapat diterima oleh instalasi farmasi sebagai prosedur pencegahan KNC.Kata Kunci: Kejadian nyaris cedera, kesalahan penyiapan obat, penelitian tindakan ABSTRACT Near miss incident initiated by dispensing errors in hospital pharmacy unit occurred in two consecutive months, in January and February 2015. This study aims to identify the causal root problem of dispensing error into near miss incidence, identify alternative solutions, and select and implement the chosen solutions. The research method was action research using mixed methods starting from diagnosis, reconnaissance, action planning, action, and evaluation. Problem diagnose was near miss incident initiated by dispensing errors. Reconnaissance phase was done by collecting data through interviews, focus group discussion, observation, and document study. Unstructured interview was involving 6 people consisting of managers, physicians, and staffs. Focus group discussion was involving hospital manager, the management, and the head of pharmacy unit.
