Medical physics practice guideline 4.b: Development, implementation, use and maintenance of safety checklists

Abstract: The American Association of Physicists in Medicine (AAPM) is a nonprofit professional society whose primary purposes are to advance the science, education, and professional practice of medical physics. The AAPM has more than 8000 members and is the principal organization of medical physicists in the US. The AAPM will periodically define new practice guidelines for medical physics practice to help advance the science of medical physics and to improve the quality of service to patients throughout the US. “…Guidelines for the development and implementation of checklists have been recently updated. 21 It is interesting to note that the top failure modes in our analysis for ocular treatments do not entirely mirror the general risk assessment in proton therapy. AAPM Task Group Report 275 tabulated some of the failure modes most relevant to proton therapy.…”

“…Where automation or forcing measures could not be taken due to human interactions with patients, multiple intermediate actions such as time‐out cross‐checks were found effective in preventing potential errors, particularly immediately prior to treatment delivery. “…Since this risk analysis was performed, a shorter online checklist was created. This checklist includes only the most important reminders in a concise format and better adheres to the recommendation of Medical Physics Practice Guideline 4.a for the development of checklists 4 . It is embedded with “yes”/“no” or fill‐in‐the‐blank answer prompts (see Figure 1).…”

“…The process for infusion should be developed and tested as part of commissioning, and may benefit from the use of checklists. 26 Steps to address extravasation can also be included on the checklist, and should follow guidelines in place at the treating center. Some commonly recommended steps including pausing the infusion, elevating the injection site, and applying warm compresses on the infusion site to accelerate vasodilation and blood flow to the tissues allowing dispersion of the drug from the area.…”

“…With vial or syringe, there is a need to monitor for leaks and for extravasation during the infusion. The process for infusion should be developed and tested as part of commissioning, and may benefit from the use of checklists 26 . Steps to address extravasation can also be included on the checklist, and should follow guidelines in place at the treating center.…”
