Mechanism of Action of Mesenchymal Stem Cell-Derived Exosomes in the Intervertebral Disc Degeneration Treatment and Bone Repair and Regeneration

Abstract: Exosomes are extracellular vesicles formed by various donor cells that regulate gene expression and cellular function in recipient cells. Exosomes derived from mesenchymal stem cells (MSC-Exos) perform the regulatory function of stem cells by transporting proteins, nucleic acids, and lipids. Intervertebral disc degeneration (IDD) is one of the main causes of low back pain, and it is characterized by a decreased number of nucleus pulposus cells, extracellular matrix decomposition, aging of the annulus fibrosus,… Show more

“…With the identification of the natural existence of stem/progenitor cells in degenerate intervertebral disc ( 114 ), possible approaches were conceived to overcome the endogenous repair failure in natural IDD process ( 77 , 101 , 116 , 115 ). In the most recent papers, therapies based on MSC-derived exosomes ( 116 , 117 , 118 ) or notochordal cells ( 119 , 120 ) were regarded as novel strategies for ameliorating IDD, with a relative preference for exosomes according to the number of articles.…”

“…[112] A large amount of data suggests that MSC-EVs maintain ECM homeostasis by upregulating the expression of ECM-related molecules. [113] BM-MSC-derived exosomes carrying circ_0050205 promote NPC proliferation and produce a healthy ECM in degenerated NPCs. [114] Similarly, exosomes from adipose tissue-derived MSCs increased and decreased the expression of type II collagen and MMP-13, respectively, thereby promoting ECM formation in degenerative intervertebral discs.…”

“…Further, they also contain various paracrine mediators, which are considered as a potential cell-free therapy for IVDD [57]. Exosomes derived from MSCs contain of a variety of regulatory factors that inhibit apoptosis, inhibit ECM degradation, downregulate inflammation, and promote chondrogenic differentiation, which could prevent IVDD [84] (Tables 1 and 2).…”
