The scientific research is dedicated to studying of treatment problems of mesial malocclusion with the anterior mandible shift among preschool aged children (in the period of temporary dentition). The point of the study is to demonstrate the effect of treating mesial malocclusion associated with the mandible anterior shift and reverse overjet in a clinical case description. During the research of a clinical case long term results were analyzed. The method of treating mesial malocclusion associated with the mandible anterior shift and reverse overjet included usage of the original appliance. The appliance was also supposed to lead to harmonic development of a child’s oro-facial area. The analysis showed advantages of the suggested original orthodontic appliance. The malocclusion was corrected in a short time (3 months), that let us develop physiological jaws and teeth arches relationships and to resolve mandible side shifting before the central incisors erupted. During the eruption permanent incisors take their place and establish the proper overjet, that creates physiological conditions for harmonic oro-facial development. The result is stable over time. The appliance is easy to use and to adapt by children and has a good fixation in oral cavity. Thus, the suggested orthodontic appliance could be recommended for treatment of mesial malocclusion associated with the mandible anterior shifting among preschool aged children.

Резюме. Порушення в розвитку зубощелепового апарату завдають шкоди не тільки здоров’ю дитини, але і негативно впливають на її адаптацію в соціальному середовищі. Велике значення у розвитку зубощелепних аномалій (ЗЩА) має спадковість. Вивчення спільних зв’язків між пальцевими малюнками та проведення генеалогічного аналізу й ЗЩА може допомогти в ранньому виявленні осіб із генетичною схильністю до аномалій. Мета дослідження – визначити роль генеалогічних та дерматогліфічних досліджень для скринінгу осіб, схильних до зубощелепних аномалій. Матеріали і методи. Для вирішення поставлених завдань проведені дослідження у 15 осіб від 19 до 74 років, що становили широке коло родичів пробанда З зі складанням родоводу в трьох поколіннях: батьки, діти й онуки. Результати досліджень та їх обговорення. Для родичів пробанда характерними візерунками були ульнарні петлі та/або дуги на пальцях правої та лівої рук. Клініко-генеалогічний аналіз членів сімей пробанда виявив, що в усіх випадках захворювання спостерігається в кожному поколінні; наявність її у одного з батьків; кожний хворий має хоча б одного родича з тією ж самою ознакою. Про аутосомно-домінантний тип успадкування свідчить поширеність цієї патології по вертикалі в обох статей. ЗЩА бувають у кожному поколінні родоводу. Співвідношення хворих хлопчиків і дівчат однакове. Імовірність народження дитини із ЗЩА, якщо хворий один з батьків становить 50 %. Висновки. Дерматогліфічний аналіз в комплексі з генеалогічними дослідженнями може бути використано для скринінгу спадкових захворювань серед населення з метою ранньої діагностики. Ефективним підходом до профілактики спадкових захворювань зубощелепних аномалій є медико-генетичне консультування, в результаті якого пацієнти можуть отримати відомості про успадкування патології, ймовірності її розвитку.

Predicting the Development of Maxillofacial Anomalies in Young School-Age Children

Recent studied have pointed out an increase in the prevalence of maxillofacial anomalies, and the rates of their occurrence rate in children of different regions of Ukraine is an evidence of the polyetiologic nature of this pathology. The formation and development of the human body is predetermined by a number of factors including parental genetic characteristcs, the state of mother's health, and the course of pregnancy that, in turn, is amenable to the influence of some external factors, e.g. social, hygienic, environmental and others. The purpose of the work was to develop an algorithm and a technique for predicting the development of maxillofacial anomalies in young school-age children. The study of the health status of children with maxillofacial anomalies included a retrospective analysis of conditions during perinatal development by reviewing data from obstetric and perinatal records (74 children). The control group consisted of 67 children who had no maxillofacial anomalies. Social, hygienic, environmental, medical and biological factors were investigated in the families having the children with and without maxillodental anomalies. On the basis of the data collected, a matrix for predicting the development of maxillofacial anomalies in young school-age children was compiled. The retrospective analysis of 74 children has shown that the risks we predicted are confirmed in 92.4% of cases. The remaining 7.6% can be explained by the combination of various adverse factors in the postnatal period, such as bad habits, modes of baby's nutrition, etc. The results obtained have demonstrated the highest values of relative risks are due to the factors known as leading in the development of maxillodantal anomalies: heredity (34.6 and 34.3), large foetus (35.3). Applying this prediction algorithm to each newborn makes it possible to evaluate the risks of maxillodental anomalies and to carry out preventive and corrective measures in early period to prevent the onset and development of maxillofacial anomalies.
