Nowe metody w diagnostyce i terapii/New methods in diagnostic and therapy WstępZakrzepica żył głębokich (ZŻG, ang. deep vein thrombosis) (ryc. 1.), będąca obok zatorowości płucnej (ZP, ang. pulmonary embolism) (ryc. 2.) objawem żylnej choroby zakrzepowo-zatorowej (ŻChZZ, ang. venous thromboembolism), pozostaje poważnym problem współczesnej medycyny.Według definicji ZŻG to powstanie zakrzepu w żyłach pod powięzią głęboką kończyny (w ok. 90% dolnej), ale tradycyjnie określenie to obejmuje również zakrzepicę żył biodrowych oraz żył przeszywających [1]. Biorąc pod uwagę miejsce powstania zakrzepu w układzie żylnym kończyn dolnych, ZŻG można podzielić na dystalną (żył piszczelowych i strzałkowych) oraz proksymalną -stanowiącą więk-sze zagrożenie dla zdrowia i życia, a dotyczącą żyły podkolanowej, udowej i biodrowej. Masywna, ostra zakrzepica żylna obejmująca większość żył odprowadzających krew z kończyny może przyjąć postać bolesnego obrzęku białe-go (ang. phlegmasia alba dolens) lub siniczego (ang. phlegmasia cerulea dolens) [1].Dokładne ustalenie liczby zachorowań na ZŻG jest trudne, ale na podstawie badania VITAE (Venous Thrombo-Embolism Impact Assessment Group in Europe) oszacowano, że w sześciu krajach Unii Europejskiej (ok. 310 mln mieszkań-ców) rocznie stwierdza się ponad 450 tys. przypadków objawowej ZŻG [2]. W Stanach Zjednoczonych na objawową ŻChZZ choruje ponad 600 tys. pacjentów rocznie [3].Ekstrapolując te wyniki na populację mieszkańców Polski, należy założyć, że objawowa ZŻG występuje rocznie u 57 tys. pacjentów [1]. Biorąc pod uwagę starzenie się społeczeństwa oraz fakt, że częstość występowania ZŻG wzrasta wraz z wiekiem chorych, można się spodziewać, że problem ZŻG i jej powikłań będzie narastał.Nierozpoznana proksymalna ZŻG nawet w 1/4 przypadków kończy się zgonem spowodowanym ZP [4]. Włą-czenie w odpowiednim czasie leczenia przeciwzakrzepowego w dużym stopniu redukuje śmiertelność. Niestety, nawet u 50% osób z ZŻG (szczególnie obejmującą żyły biodrowe i udowe) rozwija się późne powikłanie w postaci zespołu pozakrzepowego (ZPZ, ang. postthrombotic syndrome) (ryc. 3.), a u ok. 5% pacjentów nadciśnienie płucne [5][6][7]. Ponadto w niemal 20% przypadków w ciągu 2 lat dochodzi do nawrotu zakrzepicy [8]. Predysponowani do tego są szczególnie pacjenci, u których nie nastąpiło całkowite rozpuszczenie skrzepliny [9]. Następ-stwa ZŻG wiążą się ze znacznym pogorszeniem jakości życia chorych i absencją w pracy, co stanowi obciążenie finansowe dla systemu ubezpieczeń [10].

Thrombolysis versus thrombectomy in acute deep vein thrombosis reviewDeep vein thrombosis (DVT) is a significant health problem leading to the admission of over 250,000 Americans each year. Its most serious acute complication, namely pulmonary embolus kills approximately 100,000 each year, and is the third most common cardiovascular related mortality after myocardial infarction and stroke. Chronic leg problems following DVT include leg heaviness, tiredness, cramping and ulceration. These are termed the post-thrombotic syndrome. The current standard therapy of anticoagulation has changed little over 50 years. It does not remove or destroy thrombus, relying instead on the bodies own fibrinolytic mechanisms to do so. DVT needs to be more accurately categorized on an anatomical basis, and for a variety of reasons, the area of most importance is the iliofemoral region. The rationale for active, rather than passive, thrombus removal relies on multiple observations that, by doing so improves luminal patency, restores valvular function and has the potential to reduce the severity of post-thrombotic syndrome. Techniques of thrombus removal include catheter-directed thrombolysis, mechanical thrombectomy, and various combinations of both (pharmacomechanical catheter-directed thrombolysis). Although there no few direct trials comparing these directly, there is a reasonable body of evidence demonstrating their efficacy.
